西地那非+高频振荡通气治疗新生儿持续性肺动脉高压的临床效果分析 被引量:2

摘要 目的观察分析西地那非+高频振荡通气治疗新生儿持续性肺动脉高压的临床效果。方法选择在该院进行治疗的新生儿持续性肺动脉高压患儿68例,具体选择时间为2018年7月-2019年7月,随机分为常规组(n=34)和研究组(n=34),常规组行西地那非治疗,研究组行西地那非与高频震荡通气的联合治疗,比较两组患儿肺动脉压及血气指标。结果治疗后研究组患儿的肺动脉压为(30.04±5.12)mmHg,研究组肺动脉压水平明显低于常规组,差异有统计学意义(t=7.120,P<0.05);研究组治疗有效例数为32例,治疗有效率为94.12%,研究组治疗有效率明显高于常规组,差异有统计学意义(χ^2=4.221,P<0.05);研究组患儿的OI水平为(124.58±22.41),PaCO2水平为(37.84±5.36)mmHg,SaO2水平为(98.26±4.42)%,PaO2水平为(86.53±9.03)mmHg,研究组患儿的OI、SaO2及PaO2水平明显高于常规组,差异有统计学意义(t=4.760、4.948、4.803、6.185,P<0.05)。结论将西地那非与高频振荡通气联合应用于新生儿持续性肺动脉高压患儿的治疗中,能使患儿的肺动脉压水平显著降低,并对治疗有效率进行提高,患儿的OI、SaO2、PaCO2及PaO2等血气指标也得到了明显的改善,在患儿的预后恢复中发挥促进作用,值得推广使用。 Objective To observe and analyze the clinical effect of sildenafil + high-frequency oscillatory ventilation in the treatment of neonatal persistent pulmonary hypertension. Methods 68 children with persistent pulmonary hypertension in the hospital were selected. The specific selection time was from July 2018 to July 2019. They were randomly divided into the routine group(n=34) and the study group(n=34). The routine group was treated with sildenafil. The study group was treated with sildenafil and high-frequency oscillatory ventilation. The pulmonary artery pressure and blood gas index were compared between the two groups. Results After treatment, the pulmonary artery pressure of the study group was(30.04±5.12) mmHg. The pulmonary artery pressure level of the study group was significantly lower than that of the conventional group, the difference was statistically significant(t=7.120,P<0.05). The effective number of treatment in the study group was 32 cases. The effective rate was 94.12%. The treatment efficiency of the study group was significantly higher than that of the conventional group, and the difference was statistically significant(χ^2=4.221,P <0.05). The OI level of the study group was(124.58±22.41) and the level of PaCO2 was(37.84±5.36) mmHg, SaO2 level was(98.26±4.42)%, and PaO2 level was(86.53±9.03) mmHg. The levels of OI, SaO2 and PaO2 in the study group were significantly higher than those in the conventional group, the difference was statistically significant(t =4.760,4.948, 4.803,6.185,P<0.05). Conclusion sildenafil with high frequency oscillatory ventilation combined application in the treatment of newborn children with persistent pulmonary hypertension, to significantly reduce the patient’s level of pulmonary artery, and the treatment efficiency are improved, patients of OI, SaO2, PaCO2 and PaO2, blood gas index also got obvious improvement, play a promoting role in the prognosis of patients in recovery,worthy of promotion of use.
