湖州地区老年女性患者用药依从性及影响因素分析

Analysis on medication compliance and influencing factors among elderly female patients in Huzhou

目的分析湖州地区老年女性患者用药依从性及影响因素。方法回顾性分析2017年3月-2019年3月收治的226例年龄≥65岁的老年女性患者临床相关资料,探讨老年女性患者一般资料、用药情况及常见老年综合征对老年女性患者用药依从性的影响。结果226例老年女性患者中,完全遵医嘱用药164例,≥80%的时间遵医嘱用药53例,<80%的时间遵医嘱用药9例。不同年龄、婚姻状况、教育程度及居住情况老年患者的用药依从性对比,差异无统计学意义(P>0.05)。用药≤5种的老年女性患者用药依从性较用药6~10种及以上的老年女性患者高,药物管理能力良好的老年女性患者用药依从性较药物管理能力较差的老年女性患者高,差异有统计学意义(P<0.05)。存在1次跌倒史、焦虑、抑郁、衰弱前期、日常生活能力(ADL)受损的老年女性患者用药依从性较无跌倒史、焦虑、抑郁、衰弱前期、ADL受损的老年女性患者用药依从性低,差异有统计学意义(P<0.05)。Logistic回归分析提示:抑郁、跌倒史及ADL受损与老年女性患者用药依从性密切相关(OR>1,P<0.05)。结论湖州地区老年女性患者用药依从性有待加强,应多关注多重用药、抑郁、衰弱、ADL受损的患者,加强患者用药依从性管理。

Objective To analyze medication compliance and influencing factors among elderly female patients in Huzhou.Methods The clinical related data of 226 elderly female patients aged 65 years old or above treated from March 2017 to March 2019 were retrospectively analyzed.The effects of general data,medication situation,and common geriatric syndrome on medication compliance of elderly female patients were analyzed.Results Among 226 elderly female patients,164 elderly female patients strictly followed prescriptions according to the doctors’advice,53 elderly female patients followed prescriptions for at least 80%of the time,and 9 elderly female patients followed prescriptions for less than 80%of the time.There was no statistically significant difference in medication compliance among elderly female patients in different age groups,marital statuses,educational backgrounds,and inhabiting information(P>0.05).Medication compliance of elderly female patients taking at least 5 drugs was higher than that of elderly female patients taking 6-10 or more drugs,medication compliance of elderly female patients with good drug management ability was higher than that elderly female patients with poor drug management ability(P<0.05).Medication compliance of elderly female patients with fall history for once,anxiety,depression,debilitation at earlier stage,and activity of daily living(ADL)impairment was statistically significantly lower than those of elderly female patients without fall history,anxiety,depression,debilitation at earlier stage,and ADL impairment(P<0.05).Logistic regression analysis showed that depression,fall history for once,and ADL impairment were closely correlated with medication compliance of elderly female patients(OR>1,P<0.05).Conclusion Medication compliance of elderly female patients needs to be improved.More attention should be paid to the patients with multiple medications,depression,debilitation,and ADL impairment,and management of medication compliance should be strengthened.
