2018年湖南省祁阳县96例登革热病例临床及实验室特征分析

Clinical and laboratory characteristics of 96 cases of dengue fever in Qiyang County,Hunan Province,China in 2018

目的分析2018年湖南省祁阳县96例登革热病例的临床及实验室特征。方法回顾性分析2018年9月湖南省永州市祁阳县各医院收治的96例登革热住院病例的流行病学特征、临床特点及实验室检查结果。结果流行病学调查显示,96例中无输入性病例。临床特征为发热(94. 79%)、乏力(76. 04%)、全身酸痛(48. 96%)、腹泻(32. 29%)、关节痛(32. 29%)、腰痛(30. 21%)、呕吐(28. 13%)、头痛(17. 71%)、皮疹(14. 58%)、皮肤发红(13. 54%)、出血(13. 54%)、眼眶痛(3. 13%),另外伴有畏寒、肌肉疼痛,恶心、喉痛、流鼻涕、鼻塞、纳差、咳嗽、头晕、食欲下降、口苦等症状。白细胞下降和血小板计数减少病例分别占78. 13%和55. 21%;患者均有不同程度的肝功能(54. 17%)、肾功能(20. 83%)和心肌(36. 45%)损伤。96例病例血清分型结果提示,均为DENV-2感染。结论2018年湖南省永州市祁阳县登革热病例均为本土病例,流行型别为DENV-2。部分登革热患者除了表现出典型的临床症状,还有不同程度的肝功能、肾功能或心肌受损,提示患者一旦出现上述情况,医疗部门应高度重视,防控病情进一步恶化,防止重症或死亡发生。

Objective To analyze the clinical and laboratory characteristics of 96 cases of dengue fever in Qiyang County,Hunan Province,China in 2018. Methods The epidemiological characteristics,clinical characteristics and laboratory examination results of 96 patients with dengue fever,hospitalized in various hospitals in Qiyang County in September2018 were retrospectively analyzed. Results Epidemiological investigation showed no imported cases in the 96 cases.The clinical features included fever(94. 79%),fatigue(76. 04%),general soreness(48. 96%),diarrhea(32. 29%),arthralgia(32. 29%),lumbago(30. 21%),vomiting(28. 13%),headache(17. 71%),rash(14. 58%),skin redness(13. 54%),hemorrhage(13. 54%)and pain in rim of the eye. In addition,there were chills,muscle pain,nausea,sore throat,runny nose,nasal obstruction,poor appetite,cough,dizziness,loss of appetite,bitterness of mouth and other symptoms. Decreases in leukocyte and platelet counts accounted for 78. 13% and 55. 21% of total cases respectively.There were different degrees of lesion in liver function(54. 17%),renal function(20. 83%)and myocardium(36. 45%).Serotyping result indicated that all the 96 cases were infected with DENV-2. Conclusion All the cases of dengue fever in Qiyang County,Yongzhou City,Hunan Province in 2018 were local cases,of which the epidemic type was DENV-2.Partial patients with dengue fever showed typically clinical manifestations as well as different degrees of lesions in liver function,renal function or myocardium,suggesting that once the above situation occurs,the medical department should attach great importance to prevent and control the further deterioration of the disease and prevent the occurrence of severity or death.
