微视频指导下早期康复训练在心脏瓣膜置换术患者中的应用 被引量:4

Effects of Early Rehabilitation Training on Cardiac Rehabilitation of the Patients with Cardiac Valve Replacement Under the Guidance of Micro-video

摘要 目的探讨微视频指导下早期康复训练对心脏瓣膜置换术患者心脏康复效果的影响。方法将2017年1月至2019年1月医院收治的90例行心脏瓣膜置换术治疗的患者随机分为45例对照组与45例观察组,对照组采用常规康复训练,观察组则应用微视频指导下早期康复训练,测定两组干预前后心脏功能指标:左室射血分数(left ventricular ejection fractions,LVEF)、二尖瓣舒张早末期血流峰值比(E/A)、冠脉血流储备分数(FFR)变化;使用Fugl-Meyer量表评价两组干预前后的运动功能,并测定两组干预前后6 min步行试验距离(6MWT)、Barthel指数;出院后进行随访,统计1月内心脏不良事件发生率。结果观察组干预后LVEF、E/A、FFR、6MWT、Fugl-Meyer评分及Barthel指数均高于对照组(P<0.05);在心脏不良事件发生率方面,观察组为6.67%,低于对照组的22.22%(P<0.05)。结论微视频指导下早期康复训练可有效改善心脏瓣膜置换术患者心功能,并能提高运动耐力,降低心脏不良事件发生率。 Objective To investigate the effects of early rehabilitation training on cardiac rehabilitation of the patients with cardiac valve replacement under the guidance of Micro-video Methods From January 2017 to January 2019,ninety patients with cardiac valve replacement in hospital were randomly divided into control group(45 cases)and observation group(45 cases).The control group was given routine rehabilitation training while the observation group was given early rehabilitation training guided by micro-video.The cardiac function indexes[left ventricular ejection fraction(LVEF),mitral valve diastolic early-end blood flow peak ratio(E/A),and coronary fractional flow reserve(FFR)]were measured in the two groups before and after the intervention.The motor function in the two groups before and after intervention was evaluated by use of the Fugl-Meyer scale.The 6 min walking test(6MWT)and Barthel index were measured in the two groups before and after the intervention.The follow-ups were performed after discharge to calculate the incidence rate of adverse cardiac events within 1 month.Results The LVEF,E/A,FFR,6MWT,Fugl-Meyer score and Barthel index were higher in the observation group than those in the control group(P<0.05).The incidence rate of adverse cardiac events in observation group was lower than that in control group(6.67%vs 22.22%)(P<0.05).Conclusion Early rehabilitation training guided by micro-video can effectively improve cardiac function and exercise tolerance as well as reduce the incidence rate of adverse cardiac events in patients with cardiac valve replacement.
