百令胶囊治疗早期糖尿病肾病的疗效及对血管内皮功能和微炎症状态的影响

Effect of Bailing Capsule on early diabetic nephropathy and its influence on vascular endothelial function and microinflammatory state

目的评估早期糖尿病肾病(EDN)患者接受百令胶囊辅助治疗的近期疗效,以及对机体血管内皮功能、微炎症状态的影响。方法回顾性选取2018年9月至2019年8月间在鞍山市鞍钢集团总医院首诊早期糖尿病肾病的患者140例作为EDN组,根据治疗方案分为接受常规西医治疗的对照组78例和接受百令胶囊联合常规西医治疗的百令胶囊组62例。治疗3个月后,评估两组患者的近期疗效以及血管内皮功能指标[内皮依赖性舒张功能(FMD)、内皮非依赖性舒张功能(NMD)、内皮素-1(ET-1)、一氧化氮(NO)、可溶性血管细胞粘附分子-1(sVCAM-1)]、炎症相关指标[巨噬细胞清道夫受体A(SR-A)、C-C基序趋化因子2(CCL2)、肿瘤坏死因子α(TNF-α)、白介素-1(IL-1)]水平的变化情况。结果治疗后,百令胶囊组患者的治疗有效率96.77%,高于对照组患者的87.18%(P<0.05);血管内皮指标FMD、NMD、NO的水平均高于对照组患者,ET-1、sVCAM-1的水平低于对照组患者;炎症相关指标SR-A、CCL2、TNF-α、IL-1水平均低于对照组患者(P<0.05)。两组患者治疗期间药物不良反应发生率差异无统计学意义(P>0.05)。结论EDN患者在常规治疗同时加入百令胶囊辅助治疗,可有效提升患者近期疗效且有助于优化机体血管内皮功能、抑制全身炎症反应,是一种有效的治疗方案。

Objective To evaluate the short-term efficacy of Bailing capsule adjuvant therapy in patients with early diabetic nephropathy(EDN),and to detect its effect on vascular endothelial function,micro-inflammatory state.Methods 140 cases of EDN patients in our hospital from September 2018 to August 2019 were chosen as research subjects,according to different treatment methods all patient were divided into Control group(n=78,underwent routine western medicine treatment),Bailing capsule group(n=62,underwent bailing capsule combined with routine western medicine treatment).3 months after treatment,the short-term efficacy,vascular endothelial function indexes[endothelial function(FMD),endothelium-independent diastolic function(NMD),endothelin-1(ET-1),nitric oxide(NO),soluble vascular cell adhesion molecule-1(sVCAM-1)]and inflammation-related indexes[serum receptor A(SR-A),C-C motif chemokine 2(CCL2),tumor necrosis factor alpha(TNF-α),interleukin-1(IL-1)]were evaluated.Results After treatment,the effective rate of Bailing capsule group was higher than that of Control group(P<0.05).Levels of FMD,NMD and NO were higher than those of Control group,levels of ET-1 and sVCAM-1 were lower than those of Control group.Levels of inflammation-related indexes SR-A,CCL2,TNF-αand IL-1 were lower than those of Control group(P<0.05).There were no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion Adding Bailing capsule to the routine treatment of EDN patients can effectively improve the short-term curative effect,optimize the vascular endothelial function and inhibit systemic inflammation.It is an effective treatment.
