摘要 目的:利用医学上常用的Meta分析方法,综合分析和探讨铅暴露对儿童智力发育的影响。方法:按照中英文“铅”“儿童”“智力”等篇名或主题词,通过计算机检索公开发表在各数据库(中国知网、维普、万方、PubMed和Medline)上的关于铅暴露对儿童神经及智力影响的文献,对纳入文献质量评价后,并采用Review Manager 5.3对所纳入文献进行异质性分析、敏感性分析和分层分析。结果:按照预先设计的筛选标准共收集到14篇文献(英文1篇,中文13篇),经分析显示各研究之间存在中高度异质性(I 2>50%),选取随机效应模型对总智商(FIQ)、语言智商(VIQ)和操作智商(PIQ)的均数差(MD)和95%置信区间分析,结果均显示研究结局P<0.001且在95%置信区间内,按发表时间顺序和样本量从小到大对PIQ指标,以及样本量从大到小对VIQ指标进行依次剔除,结果都显示合并效果的可靠性,说明合并研究结果有统计学意义。最后将研究现场进行亚组分析,发现研究现场是各指标异质性来源的影响因素。结论:铅暴露对儿童神经和智力发育存在一定负面影响。 Objective:Using the Meta-analysis methods commonly applied in medicine to comprehensively analyze and explore the effectsofleadexposureonchildren'smentaldevelopment.Methods:BasedonChineseorEnglishkeywords"lead""child""intellectual",articles about the influence of lead exposure on children's nerves or intelligences were searched though various databases(CNKI,Weipu,Wanfang,PubMed and Medline).After a quality evaluation of the literature,the heterogeneity analysis,sensitivity analysis and stratification analysis of the included literature were carried out using Review Manager 5.3.Results:A total of 14 articles(1 article in English and 13 articles in Chinese)were collected according to the pre-designed screening criteria.The analysis showed that there was a high degree of heterogeneity among the studies(I 2>50%),and the random effects model was selected to analyze the mean difference(MD)and 95%confidence interval of full intelligence quotient(FIQ),verbal intelligence quotient(VIQ),and performance intelligence quotient(PIQ).Results showed that the study outcome was P<0.001 and within the 95%confidence interval.The PIQ indicators were eliminated from time order of publication and from large to small in accordance with the sample size,and the VIQ indicators were eliminated from large to small in accordance with the sample size.The results showed the reliability of the combined effect,indicating that the combined results were statistically significant.Finally,a subgroup analysis of the research site was carried out,and it was found that the field study was the influencing factor of the heterogeneity of each indicator.Conclusion:Lead exposure has a negative impact on children's neurological and intellectual development.
