1020张门急诊芪苈强心胶囊处方专项点评与分析

Review and Analysis on 1020 Prescription of Qiliqiangxin Capsule in Outpatient and Emergency Departments

目的:了解上海中医药大学附属龙华医院(以下简称“我院”)门急诊芪苈强心胶囊的使用情况,以促进中成药的合理应用,提高处方质量。方法:随机抽取2017年12月1日至2018年12月1日我院门急诊使用芪苈强心胶囊的处方1020张,使用我院自主研发的处方分析系统PA系统,对功能主治、用法与用量以及联合用药等进行专项点评,并对结果进行分析,评估处方合理性。结果:1020张门急诊芪苈强心胶囊处方中,不合理处方共172张(占16.9%);其中,不规范处方2张(占1.2%),用药不适宜处方76张(占44.1%),超常处方94张(占54.7%);不合理处方主要涉及违反使用禁忌,药物选择与诊断、证型不符以及重复用药等问题。结论:门急诊芪苈强心胶囊的临床使用存在一定问题,需加强管理,促进合理用药。

OBJECTIVE:To investigate the application of Qiliqiangxin capsule in outpatient and emergency departments in Affiliated Longhua Hospital of Shanghai University of Traditional Chinese Medicine(hereinafter referred to as“our hospital”),so as to promote the rational application of traditional Chinese medicine and improve the quality of prescriptions.METHODS:1020 prescriptions applied with Qiliqiangxin capsule in outpatient and emergency departments in our hospital from Dec.1st,2017 to Dec.1st,2018 were randomly extracted,specialized review on functional indications,usage and dosage and drug combination were conducted by adopting the self-designed prescription-analysis system-PA,the results were collected and analyzed to assess the rationality of prescriptions.RESULTS:Of the 1020 prescriptions of Qiliqiangxin capsule,there were 172 irrational prescriptions(16.9%);including 2 nonstandard prescriptions(1.2%),76 prescriptions with inappropriate drug application(44.1%),94 abnormal prescriptions(54.7%);the irrational prescriptions were mainly involving issues including violations of contraindications,inconsistency of drug selection with diagnosis and syndromes,repeated drug application,etc.CONCLUSIONS:There are certain problems in the clinical application of Qiliqiangxin capsule in outpatient and emergency departments,management needs to be strengthened to promote rational drug application.
