动物布鲁氏菌病5种血清学检测方法的应用比较和分析评价 被引量:10

摘要 [目的]探讨当前动物布鲁氏菌病实验室快捷准确的诊断方法。[方法]对137份布鲁氏菌病虎红平板凝集试验(RBT)阳性血清分别用试管凝集试验(SAT)、补体结合试验(CFT)、酶联免疫吸附试验(ELISA)和荧光偏振试验(FPA)等5种不同的血清学检测方法进行检测,分析、评价不同检测方法的特异性、敏感性、假阳性率(误诊率)、假阴性率(漏诊率)、符合率、Kappa值等试验指标。[结果] I-ELSIA、CELISA、FPA敏感性高、特异性好、漏诊率低、符合率高、Kappa值均较理想,CFT特异性好、敏感性低、漏诊率高、符合率差、kappa值小于0.75。[结论]对于动物布鲁氏菌病实验室诊断,要结合实情选择检测方法,伴随目前检测技术的进步和发展,ELSIA和FPA方法值得应用推广,将对防控动物布鲁氏菌病意义重大。 [Objective] In order to discuss and seek the current methods for fast and accurate diagnosis of animal brucellosis in laboratory. [Methods] 137 serum samples detected brucellosis-positive by rose bengal plate test(RBT) were tested and analyzed with 5 different serological testing methods, namely standard tube agglutination test(SAT), complement fixation test(CFT), enzyme-linked immunosorbent assay(ELISA), and fluorescence polarization test(FPA),etc. On such a basis, the specificity, sensitivity, false positive rate(misdiagnosis rate), false negative rate(rate of missed diagnosis), coincidence, Kappa value and other experimental indicators of these test methods were further evaluated. [Results] The methods of I-ELSIA, CELISA and FPA showed high sensitivity, good specificity, low rate of missed diagnosis, high coincidence, and more ideal Kappa value;CFT method showed good specificity, low sensitivity, high rate of missed diagnosis,poor coincidence, and the kappa value was smaller than 0.75. [Conclusion] Appropriate test methods should be selected for laboratory diagnosis of animal brucellosis according to actual situation. In view of the advancing testing technologies today, further application of ELSIA and FPA will be both worthwhile and significant to the prevention and control of animal brucellosis.
