新型冠状病毒肺炎疫情期间儿童专科医院发热门诊应急处理机制的建立与实施

Establishment and implementation of emergency response system for fever clinic in children's hospital during the epidemic period of novel coronavirus pneumonia

目的探讨新型冠状病毒肺炎流行期间儿童专科医院发热门诊应急处理机制的建立与实施效果。方法通过建立有效的应急指挥系统,细化发热门诊区域设置,实行三级预检分诊,合理调配人力资源,强化岗前培训,制订可操作性强的工作流程和预案,规范防护用品与医疗物资管理,做好一线工作人员的心理支持等方法,保证发热门诊工作顺利开展。结果2020年1月30日至2月29日全院共接诊发热患儿1163例,其中新型冠状病毒肺炎疑似病例9例,确诊病例3例,无漏诊,实现了院内零感染。岗前培训后,发热门诊护理人员新型冠状病毒肺炎相关理论知识、个人防护技能成绩显著提高(P<0.01);在进入发热门诊前,护理人员的焦虑、恐怖因子得分高于中国成人常模(P<0.01),通过心理干预,进入发热门诊1个月后,恐怖因子得分降低,与常模差异无统计学意义(P>0.05)。结论应急处理机制的建立与实施保障了儿童专科医院发热门诊工作的顺利开展。

Objective Discuss the establishment and implementation of emergency response system during the epidemic period of COVID-19.Methods In order to keep the smooth work of fever clinic,a set of effective emergency response system was established,including establishing an emergency commanding system,refining the division of area of fever clinic,carrying out three-level triage,allocating the manpower resources rationally,strengthening pre-job-training,implementing the practical working scheme and pre-plan,managing the personal protective equipment in a scientific way and providing the psychological support for the front-line staff,etc.Results Totally,1163 cases of fever children were seen in fever clinic from 30 January to 29 February 2020.Among them,9 cases were diagnosed as suspected,3 cases were confirmed as COVID-19,no cases were misdiagnosed,no staff infected.After pre-job training,the first batch medical staff's knowledge and personal protective skills of COVID-19 had improved significantly(P<0.01).And compared with Chinese adult norm,the factor of anxiety and fear was significantly different(P<0.01)before psychological intervention,and the factor of fear was decreased after one month psychological intervention,there was no statistically significant difference from the norm(P>0.05).Conclusion The emergency response system effectively ensure the work smooth,and make well-organized and safely in progress in the fever clinic of children's hospitals.
