激光穴位照射联合运动想象疗法治疗脑卒中肢体康复的临床疗效

Clinical Effect of Laser Acupoint Irradiation Combined with Motor Imagery Therapy on Limb Rehabilitation after Stroke

目的观察激光穴位照射联合运动想象疗法治疗脑卒中肢体康复的临床疗效。方法选取2017年6月至2018年10月,脑卒中患者90例,按随机数字表法分为激光组、想象组和联合组,每组患者30例。三组均接受常规康复训练后,激光组仅采用激光穴位照射治疗,波长1064 nm,治疗功率8~10 mW,以百会穴以及患侧的合谷、手三里、肩髃、环跳、足三里为主穴,每个穴位接受光导探针照射5 min;想象组仅采用运动想象疗法治疗;联合组采用激光穴位照射联合运动想象疗法治疗。90例患者接受4周治疗后,分别采用布伦斯特罗姆(Brunnstrom)量表评分和运动功能(Fugl-Meyer Assessment,FMA)评分评估三组患者治疗前后的肢体功能,巴氏指数(Barthel Index,BI)评估患者日常生活能力以及健康调查简表-36(the MOS item short from health survey,SF-36)评估生活质量。结果治疗后4周,三组Brunnstrom和FMA评分均明显高于治疗前(P<0.05);联合组Brunnstrom及FMA评分均高于激光组和想象组(P<0.05);治疗后4周,三组BI指数和SF-36评分均比治疗前提高(P<0.05);联合组BI指数和SF-36评分均高于激光组和想象组(P<0.05)。结论激光穴位照射联合运动想象疗法可明显改善脑卒中患者肢体功能,提高日常生活能力及生活质量,促进患者肢体康复。

Objective To observe the clinical effect of laser acupoint irradiation combined with motor imagery therapy on limb rehabilitation after stroke.Methods Ninety stroke patients from Jun.2017 to Oct.2018 were divided into the laser group,imagination group and combination group with random number table method,each consisting of 30 cases.All the patients of the three groups were provided with routine rehabilitation training.The patients of the laser group,imagination group and combination group were given laser acupoint irradiation,motor imagery therapy and the combination of laser acupoin irradiation and motor imagery therapy respectively.Each patient of each group experienced 4 weeks’treatment.The limb function(Brunnstrom)scale score and Fugl-Meyer Assessment(FMA)score,daily living ability Barthel index(BI)and life quality the MOS item short from health survey-36(SF-36)scale score of the patients of each group were compared before and after treatment.Results After the treatment,the Brunnstrom scores and FMA scores in the three groups were significantly higher than those before the treatment(P<0.05),and the Brunnstrom and FMA scores in the combination group significantly higher than those in the laser group and the imagination group(P<0.05).After the treatment,the BI index and SF-36 scores in the three groups were higher than those before the treatment(P<0.05),and the BI index and SF-36 scores in the combination group significantly higher than those in the laser group and the imagination group(P<0.05).Conclusions Laser acupoint irradiation combined with motor imagery therapy will significantly improve the limb functions,daily living ability and life quality of stroke patients and promote limb rehabilitation.
