甲状腺癌患者血清半乳糖凝集素-1、谷胱甘肽过氧化物酶3表达变化及其与疾病严重程度相关性研究 被引量:10

摘要 目的研究甲状腺癌患者血清半乳糖凝集素-1(Gal-1)、谷胱甘肽过氧化物酶3(GPX3)表达变化及其与疾病严重程度关系。方法回顾性分析2017年4月~2019年1月首都医科大学附属北京天坛医院收治的92例甲状腺癌患者临床资料,同期选择本院收治的87例甲状腺良性病变患者,采用酶联免疫法检测比较两组血清Gal-1、GPX3水平,分析甲状腺癌血清Gal-1、GPX3水平和临床参数的关系,血清Gal-1、GPX3对甲状腺癌的诊断效能,及Gal-1和GPX3的相关性。结果甲状腺癌组血清Gal-1水平高于甲状腺良性病变组,GPX3水平低于甲状腺良性病变组,差异有统计学意义(P<0.05)。淋巴结转移、Ⅲ+Ⅳ期甲状腺癌患者血清Gal-1水平高于无淋巴结转移及Ⅰ+Ⅱ期,血清GPX3水平低于无淋巴结转移及Ⅰ+Ⅱ期,差异有统计学意义(P<0.05)。ROC曲线分析显示,Gal-1、GPX3、Gal-1+GPX3对甲状腺癌诊断的曲线下面积分别为0.805、0.784、0.890,灵敏度分别为0.778、0.761、0.826,特异度分别为0.682、0.701、0.817。甲状腺癌血清Gal-1与GPX3呈负相关(r=-0.496,P<0.05)。结论血清Gal-1在甲状腺癌中的水平明显上升,GPX3则显著下降,且和淋巴结转移、肿瘤分期有直接关系,可作为甲状腺癌疾病严重程度的参考指标。 Objective To study the expression changes of serum galectin-1(Gal-1)and glutathione peroxidase 3(GPX3)in patients with thyroid cancer and their relationship with disease severity.Methods The clinical data of 92 cases of thyroid cancer admitted from April 2017 to January 2019 were analyzed retrospectively.87 cases of benign thyroid lesions admitted to our hospital were selected at the same time.Serum levels of Gal-1 and GPX3 were detected by enzyme-linked immunosorbent assay.Serum levels of Gal-1 and GPX3 were compared between the two groups.The relationship between serum levels of Gal-1 and GPX3 and clinical parameters of thyroid cancer,the diagnostic efficacy of serum Gal-1 and GPX3 for thyroid cancer,and the correlation between Gal-1 and GPX3 were analyzed.Results the serum level of Gal-1 in thyroid cancer group was higher than that in thyroid benign lesion group,and the level of GPX3 was lower than that in thyroid benign lesion group(P<0.05).The serum level of Gal-1 in lymph node metastasis and stageⅢ+Ⅳgroup was higher than that in non-lymph node metastasis and stageⅠ+Ⅱgroup,while the serum GPX3 level in lymph node metastasis and stageⅢ+Ⅳgroup was lower than that in non-lymph node metastasis and stageⅠ+Ⅱgroup(P<0.05).ROC curve analysis showed that the areas under the curve of Gal-1,GPX3,Gal-1+GPX3 in the diagnosis of thyroid cancer were 0.805,0.784 and 0.890,the sensitivity was 0.778,0.761,0.826,and the specificity was 0.682,0.701 and 0.817.The serum Gal-1 of thyroid cancer was negatively correlated with GPX3,and the r value was-0.496,P<0.05.Conclusion The level of serum Gal-1 in thyroid cancer is significantly increased,while GPX3 is significantly decreased,which is directly related to lymph node metastasis and tumor staging and can be used as a reference index for the severity of thyroid cancer.
