腹腔镜远端胃癌根治术uncut roux-en-Y吻合对2型糖尿病患者术后血糖的影响

Effect of laparoscopic distal gastrectomy by uncut roux-en-Y gastrojejunostomy on postoperative blood sugar in patients with type 2 diabetes mellitus

目的:观察腹腔镜远端胃癌根治术uncut roux-en-Y吻合对2型糖尿病(T2DM)患者术后血糖的影响。方法:收集并回顾性分析82例均拟行腹腔镜远端胃癌根治术uncut roux-en-Y吻合术式的胃癌合并T2DM患者,对比手术前后FBG、2hPG、HbA1c值及术后血糖改善情况的变化。结果:术后1、3和6个月患者的FBG、2hPG和HbA1c值均明显低于术前(P<0.05),期间血糖改善有效率比较均存在显著差异(P<0.05),其中术后6个月的血糖改善有效率明显高于术后1个月和3个月(P<0.05)。结论:腹腔镜远端胃癌根治术uncut roux-en-Y吻合术式能有效改善T2DM患者的血糖,疗效确切,值得临床进一步试用。

Objective:To observe the effect of laparoscopic distal gastrectomy by uncut roux-en-Y gastrojejunostomy on postoperative blood sugar in patients with type 2 diabetes mellitus.Methods:The data from 82 cases of patients with gastric cancer complicated with T2DM were collected and retrospectively analyzed,who underwent the laparoscopic distal gastrectomy by uncut roux-en-Y gastrojejunostomy.The changes of the FBG,2hPG,HbA1c values before and after operation and the improvement of blood sugar after operation were compared.Results:The values of FBG,2hPG and HbA1c were significantly lower at 1,3 and 6 months after operation than before(P<0.05).There were significant differences in the effective rate of blood sugar improvement at 1,3 and 6 months after operation(P<0.05)and the effective rate of blood sugar improvement at 6 months after operation was significantly higher than those at 1 and 3 months after operation(P<0.05).Conclusion:The laparoscopic distal gastrectomy by uncut roux-en-Y gastrojejunostomy can improve blood sugar in patients with T2DM with clinically definite effect and worthy of trial in clinic.
