VITEK 2 compactNH卡鉴定卫星试验阳性菌株的性能观察

摘要 目的:根据因子需求对卫星试验阳性菌株进行初步鉴定,采用VITEK 2 compact系统及NH卡进行生化鉴定,观察NH卡鉴定结果与初步鉴定结果差异情况,为临床实验室提高嗜血杆菌鉴定准确性提供依据和帮助。方法:回顾分析471株卫星试验阳性的细菌其卫星特征和VITEK2 compactNH卡鉴定详细情况。结果:①以卫星试验因子需求特征初步鉴别的流感嗜血杆菌、副流感嗜血杆菌、溶血嗜血杆菌和副溶血嗜血杆菌为基础,相应菌株NH卡鉴定一致率分别为80.5%、80.9%、0%、18.9%。②所有菌株经NH卡鉴定,80.7%的菌株获得唯一可接受结果(鉴定值>80%),19.3%的菌株鉴定值(id%)低于80%,备选条目id%之和均大于80%,所有条目分布于10种菌,均为嗜血杆菌属和近亲细菌。③NH卡严重忽略细菌溶血特征而使具有溶血能力的嗜血杆菌鉴定时出现%id大于80%,但实际不符合的结果。结论:①部分与嗜血杆菌亲缘关系较近的苛养菌会出现卫星试验阳性,不能仅根据因子需求特征鉴定嗜血杆菌,种的鉴定必须采用系统鉴定方法。②与嗜血杆菌近亲的苛养菌干扰NH卡的鉴别能力。③所有的NH卡鉴定结果均需结合细菌溶血特征进行判断或修正。 Objective:Preliminary identification of positive strains in satellite test was conducted according to factor requirements.VITEK 2 compact system and NH card were used for biochemical identification.Differences between NH card identification results and preliminary identification results were observed to provide basis and help for improving the accuracy of haemophilus identification in clinical laboratories.Methods:The satellite characteristics and identification of VITEK 2 compact NH card of 471 strains of bacteria with positive satellite test were analyzed retrospectively.Resulta:①Based on the preliminary identification of haemophilus influenzae,haemophilus parainfluenzae,haemophilus hemolytic and haemophilus parahaemolyzae by satellite test factor demand characteristics,the consistent identification rates of the corresponding strains by NH card were 80.5%,80.9%,0% and 18.9%,respectively.②All the strains were identified by NH card,80.7% of the strains obtained the only acceptable result(the identification value was>80%),19.3% of the strains obtained the identification value(id%)was lower than 80%,and the sum of alternative items id%was greater than 80%.All the items were distributed among 10 species of bacteria,all of which were haemophilus and closely related bacteria.③NH card seriously ignores the characteristics of bacterial hemolysis,resulting in the identification of haemophilus with hemolysis ability with a result that%id is greater than 80%,but the actual result is not consistent.Conclusion:①Some of the caustic bacteria closely related to haemophilus will be positive in satellite test,so haemophilus cannot be identified only according to the characteristics of factor demand,but the systematic identification method must be adopted for species identification.②The caustic bacteria closely related to haemophilus interfere with the identification ability of NH card.③All NH card identification results should be judged or corrected in combination with the characteristics of bacterial hemolysis.
