摘要 目的:探究SAA、hs-CRP和WBC联合检测在儿童感染性疾病的应用价值.方法:选择我院自2018年6月~2019年6月收治的200例感染性疾病患儿作为研究对象(设为研究组,包含细菌感染70例、病毒感染70例、肺炎支原体感染60例),另选择同期在我院进行健康体检的100例健康儿童作为参照对象(设为对照组),所有受试儿童均行血清淀粉样蛋白A(SAA)、C-反应蛋白(hs-CRP)和白细胞计数(WB C)检测,对比研究组和对照组及不同感染类型组儿童SAA、hs-CRP、WBC水平,并对各项检测指标做ROC曲线,比较其单用或联合检测诊断感染性疾病的特异性、敏感性.结果:研究组儿童SAA、hs-CRP、WBC水平均显著高于对照组(P<0.05);细菌感染组、病毒感染组儿童SAA水平对比无显著差异(P>0.05),但均显著高于肺炎支原体感染组(P<0.05);细菌感染组儿童hs-CRP、WBC水平显著高于病毒感染组、肺炎支原体感染组(P<0.05);病毒感染组、肺炎支原体感染组儿童hs-CRP、WBC水平对比无显著差异(P>0.05);SAA、hs-CRP、WBC单用检测诊断感染性疾病的特异性、敏感性对比无显著差异(P>0.05),SAA、hs-CRP、WBC三项联合检测诊断感染性疾病的特异性、敏感性显著高于任一单项检测.结论:SAA、hs-CRP和WBC联合检测在儿童感染性疾病的应用价值显著,值得推广. Objective:To explore the value of combined detection of SAA,hs-CRP and WBC in children with infectious diseases.Methods:From June 2018 to June 2019,200 children with infectious diseases were selected as the study group,including 70 cases of bacterial infection,70 cases of virus infection and 60 cases of Mycoplasma pneumoniae infection.In addition,100 healthy children who underwent physical examination in our hospital at the same time were selected as the reference object(set as the control group).All the tested children were treated with serum amylase A(SAA),C-reactive protein(hs-CRP)and white blood cell count(WBC).The SAA,hs-CRP,WBC of children in studying group and control group and different infection types were compared.The level of BC and the ROC curve of each detection index were compared to compare the specificity and sensitivity of single or combined detection in the diagnosis of infectious diseases.Results:The level of SAA,hs-CRP,WBC in observation group was significantly higher than that in control group,and there was no significant difference in SAA level between the bacterial infection group and the virus infection group,but it was significantly higher than that in the Mycoplasma pneumoniae infection group(P<0.05),and the hs-CRP and WBC level in the bacterial infection group was significantly higher than that in the virus infection group and Mycoplasma pneumoniae infection group(P<0.05).There was no significant difference in hs-CRP,WBC level between virus infection group and Mycoplasma pneumoniae infection group(P>0.05),but there was no significant difference in SAA and hs-CRP level between children infected with Mycoplasma pneumoniae and Mycoplasma pneumoniae infection group.The specificity and sensitivity of WBC in the diagnosis of infectious diseases were no significant difference(P>0.05),and the sensitivity of SAA,hs-CRP and WBC in the diagnosis of infectious diseases was significantly higher than individual detection.Conclusion:The application value of SAA,hs-CRP and WBC combination test in children with infectious diseases is significant,and it is worth to be popularized.
