摘要
目的:探讨颅内感染性疾病中脑脊液中肝素结合蛋白(HBP)水平在临床应用价值.方法:回顾性分析我院2016年1月~2018年12月期间收入颅内感染性疾病患者总计80例,依据白细胞计数是否高于10×10^9/L作为依据分为两组,白细胞计数低于10×10^9/L32例为轻症感染组,白细胞计数高于10×10^9/L 48例为重症感染组,另选取同期收入非颅内感染者30例作为正常对照组,比较三组患者预后情况.结果:重症感染组HBP、WBC、中性粒细胞、TP水平高于轻症感染组、对照组,Giu水平低于重症感染组、轻症感染组,三组差异显著(P<0.05),HBP、WBC、TP、PCT及Glu曲线下面积(AUC)分别为0.906、0.694、0.703、0.711和0.369.HBP敏感度、阳性预测值、阴性预测值均高于WBC、cPCT、sPCT水平,TP特异度高于HBP、WBC及PCT水平,差异显著(P<0.05).结论:HBP作为颅内感染中理想诊断指标,对判断感染轻重程度起着重要临床意义,且升高幅度越高则感染程度越严重,与疾病预后相关.
Objective:To explore the clinical application value of heparin binding protein(HBP)in cerebrospinal fluid of intracranial infectious diseases.Methods:A retrospective analysis of 80 patients with intracranial infectious diseases in our hospital from January 2016 to December 2018 was analyed.According to whether the leukocyte count was higher than 10×10^9/L,the patients were divided into two groups.32 patients with leukocyte count lower than 10×10^9/L were mild infection group,and 48 patients with leukocyte count higher than 10×10^9/L were in the severe infection group,and 30 patients with non intracranial infection in the same period were selected as normal control group The prognosis of the three groups were compared.Results:The levels of HBP,WBC,neutrophils and TP in the severe infection group were higher than those in the light infection group and the control group,while the level of giu in the severe infection group was lower than that in the light infection group and the severe infection group,there were significant difference among the three groups(P<0.05);The AUC of HBP,WBC,TP,PCT and Glu were 0.906,0.694,0.703,0.711 and 0.369 respectively;The sensitivity,positive predictive value and negative predictive value of HBP were higher than those ofWBC,cpct and SPCT,and the specificity of TP was higher than those of HBP,WBC and PCT(P<0.05).Conclusion:As an ideal diagnostic index of intracranial infection,HBP plays an important role in judging the severity of infection.The higher the increase,the more serious the infection is,which is related to the prognosis of the disease.
