宫颈癌根治术后继发性下肢淋巴水肿危险因素与预防策略

The risk factors of secondary lower extremity lymphedema after radical operation of cervical cancer and its preventive strategies

目的探讨宫颈癌根治术后继发性下肢淋巴水肿危险因素并制定预防策略。方法选择2017年6月至2018年6月于我院行宫颈癌根治术的80例患者为研究对象,术后发生下肢淋巴水肿20例。收集所有患者年龄、淋巴结清扫数等基本资料,分析影响其发生下肢淋巴水肿的独立危险因素,并据此制定针对性预防策略。结果年龄(OR=1.966)、淋巴结清扫数目(OR=2.032)、术后放化疗(OR=2.838)、每日站立时间(OR=1.871)是宫颈癌根治术后继发性下肢淋巴水肿的独立危险因素。结论宫颈癌根治术后继发性下肢淋巴水肿,受年龄、淋巴结清扫数目、术后放化疗、每日站立时间等多种因素的影响,可根据相关危险因素制定预防策略以降低下肢淋巴水肿风险。

Objective To explore the risk factors of secondary lower extremity lymphedema after radical operation of cervical cancer and development of preventive strategies.Methods To select 80 cases of patients who underwent radical operation of cervical cancer in our hospital from June 2017 to June 2018 as the study objects,and there was 20 cases of lower extremity lymphedema occurred after operation.The basic data such as the age of all patients and the number of lymph node dissections were collected,the independent risk factors affecting the occurrence of lower limb lymphedema was analyzed,and a targeted prevention strategy accordingly was developed.Results The age(OR=1.966),number of lymph node dissections(OR=2.032),postoperative radiotherapy and chemotherapy(OR=2.838),and daily standing time(OR=1.871)were the independent risk factors for secondary lower extremity lymphedema after radical resection of cervical cancer.Conclusion The secondary lower extremity lymphedema after radical resection of cervical cancer was affected by various factors such as the age,number of lymph node dissections,postoperative radiotherapy and chemotherapy,and daily standing time.A preventive strategy should be developed according to relevant risk factors to reduce the risk of lower extremity lymphedema.
