脑卒中康复住院费用影响因素多元线性回归分析

Analysis of Variance and Multiple Linear Regression on the Structure and Influential Factors of Rehabilitation Hospitalization Costs of Stroke Patients

目的:了解脑卒中患者康复住院费用构成及主要影响因素,为合理控制脑卒中患者康复住院费用提供参考。方法:收集江苏省某医院2015年1月—2019年5月出院的脑卒中康复住院病例1813例,采用描述性分析、非参数检验和多元线性回归分析探究脑卒中康复患者住院费用及影响因素。结果:脑卒中患者康复住院费用为(14356.82±8732.83)元,占比最高是治疗费(61.45%),其次为西药费(16.47%)、床位费(9.15%)。回归分析显示住院天数、入院时ADL分值、有无糖尿病、有无肺部感染、支付方式是住院费用的影响因素,差异有统计学意义(P<0.01)。结论:脑卒中患者康复住院费用中,治疗费占比最高,不同卒中类型住院费用无差别,缩短住院时间、控制血糖水平,防治肺部感染、改善患者入院时功能状况、完善医保支付方式是控制脑卒中患者康复住院费用的有效途径。

Objective:To understand the main influencing factors of rehabilitation and hospitalization cost of stroke patients and to provide reference for rational control of rehabilitation and hospitalization cost of stroke patients.Methods:A total of 1,813 stroke reha bilitation inpatients discharged from a certain hospital in Jiangsu since January 2015 to May 2019 were collected.Descriptive analysis,non-parametric test and multiple linear regression analysis were used to explore the influencing factors of stroke rehabilitation inpa tient costs.Results:The cost of rehabilitation and hospitalization for inpatients with cerebral apoplexy was(14,356.82±8,732.83)yu an,accounting for the highest proportion of treatment cost(61.45%),western medicine cost(16.47%)and bed cost(9.15%).The regres sion analysis showed that length of stay,ADL score at the time of admission,pulmonary infection,medical insurance type and occupa tion were the influencing factors of hospitalization expenses,and the differences were statistically significant(P<0.01).Conclusion:The treatment expenses of stroke patients accounted for the highest proportion.To shorten hospitalization time,control blood sugar lev el,prevent lung infection,improve the function level at admission and reform medical insurance payment mode are effective ways to control rehabilitation hospitalization cost of stroke patients.
