浅刺法合灸法配合康复训练对中风后痉挛性偏瘫疗效观察

Observation on the Curative Effect of Shallow Acupuncture - Moxibustion Combined with Rehabilitation Training for Spastic Hemiplegia after Apoplexy

目的:观察浅刺法合灸法配合康复训练对中风后痉挛性偏瘫的临床疗效。方法:经筛选共107例病人纳入观察。随机分两组,治疗组54例,采用浅刺针法合灸法;对照组53例,采用传统针法。两组治疗后均行康复训练。每周连续治疗5次,两组均连续治疗4周,共20次。两组均于治疗前后予改良Ashworth(MAS)分级量表评分比较患肢肌张力,Fugl-Meyer(FMA)评测量表比较其运动功能。结果:治疗前两组上下肢MAS及FMA评分均无差异。治疗后治疗组上肢MAS评分明显低于对照组(P<0.01),而下肢两组之间无差异(P>0.05),FMA评分治疗组上下肢均明显高于对照组(P<0.001)。结论:浅刺法合灸法配合康复训练能更有效地缓解中风后痉挛性偏瘫患者肌痉挛,并提高其运动功能,尤其对上肢较明显,值得临床推广。

Objective:To observe the clinical effect of shallow acupuncture-moxibustion combined with rehabilitation training on spastic hemiplegia after apoplexy. Methods: 107 patients were randomLy divided into two groups: superficial acupuncture-moxibustion and traditional acupuncture were used for treatment group(N=54) and the control group(N=53), respectively. After treatment, both groups were trained with rehabilitation techniques for 5 times a week and continuously for 20 times for 4 weeks. The muscle tension of the affected limb was compared with the modified ashworth(MAS) scale before and after treatment, and the motor function of the affected limb was compared by fugl-meyer(FMA) scale. Results: No difference in MAS and FMA scores between two groups before treatment. However, the MAS score of upper limb in the treatment group was significantly lower than that in the control group(P<0.01) after treatment.No difference between the two groups(P>0.05) for lower limbs. The score of upper and lower limbs in the treatment group was significantly higher than that in the control group(P<0.001). Conclusion: Shallow acupuncture-moxibustion combined with rehabilitation training can effectively alleviate muscle spasm and improve motor function in patients with spastic hemiplegia after apoplexy, especially for upper limbs, which is worth popularizing in clinic.
