摘要
目的探讨新生儿肺炎中心肌酶谱改变的临床意义。方法90例新生儿,按照有无肺炎分为对照组(健康新生儿,30例)和观察组(肺炎患儿,60例);观察组患儿中30例确诊为重症肺炎的患儿作为观察Ⅰ组,30例确诊为单纯性肺炎的患儿作为观察Ⅱ组。对对照组和观察组新生儿进行肌酸激酶同工酶(CK-MB)、门冬氨酸氨基转移酶(AST)、肌酸激酶(CK)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(HBDH)等心肌酶谱指标进行检测。比较观察组和对照组及观察Ⅰ组和观察Ⅱ组的心肌酶谱指标差异。结果观察组CK-MB(74.56±23.14)U/L、AST(53.22±12.23)U/L、CK(345.42±43.54)U/L、LDH(476.41±62.15)U/L、HBDH(345.25±34.65)U/L均明显高于对照组的(26.42±4.54)、(32.41±10.12)、(156.25±14.46)、(154.52±15.36)、(105.36±22.74)U/L,差异有统计学意义(P<0.05)。观察Ⅰ组的CK-MB(87.34±25.15)U/L、AST(68.61±15.64)U/L、CK(396.25±53.34)U/L、LDH(526.25±72.64)U/L、HBDH(388.54±42.37)U/L均明显高于观察Ⅱ组的(62.53±15.46)、(45.32±11.36)、(315.47±30.45)、(414.58±58.35)、(310.35±33.71)U/L,差异有统计学意义(P<0.05)。结论心肌酶谱改变对于新生儿肺炎的诊断、病情评估均有重要意义。
Objective To discuss the clinical significance of changes of myocardial zymogram in neonatal pneumonia.Methods A total of 90 neonates were divided into control group(30 healthy neonates)and observation group(60 pneumonia neonates)according to whether there was pneumonia.In the observation group,30 patients with severe pneumonia were treated as observation groupⅠand 30 patients with simple pneumonia as observation groupⅡ.Indexes of myocardial zymogram of creatine kinase isoenzyme(CK-MB),aspartate aminotransferase(AST),creatine kinase(CK),lactate dehydrogenase(LDH)andα-hydroxybutyrate dehydrogenase(α-HBDH)were detected in the control group and the observation group.The difference of myocardial zymogram was compared between observation group and control group,observation groupⅠand observation groupⅡ.Results The levels of CK-MB,AST,CK,LDH andα-HBDH were(74.56±23.14),(53.22±12.23),(345.42±43.54),(476.41±62.15),and(345.25±34.65)U/L respectively in the observation group,which were obviously higher than(26.42±4.54),(32.41±10.12),(156.25±14.46),(154.52±15.36)and(105.36±22.74)U/L in the control group,and their difference was statistically significant(P<0.05).The levels of CK-MB,AST,CK,LDH andα-HBDH were(87.34±25.15),(68.61±15.64),(396.25±53.34),(526.25±72.64)and(388.54±42.37)U/L respectively in observation groupⅠ,which were obviously higher than(62.53±15.46),(45.32±11.36),(315.47±30.45),(414.58±58.35)and(310.35±33.71)U/L in observation groupⅡ,and their difference was statistically significance(P<0.05).Conclusion Changes of myocardial zymogram are important for the diagnosis and evaluation of neonatal pneumonia.
