两种微种植支抗方法拔牙矫治的疗效对比

Effect comparison of two kinds of micro-implant anchorage methods in the orthodontic treatment with teeth extraction

目的对比使用两种不同位置的微种植钉拔牙矫治牙性骨性混合型上颌前突成年患者的疗效。方法选择适应证患者38例,按上颌磨牙区微种植钉可植入高度分为高位组(20例)和低位组(18例)。比较治疗前后各测量指标的变化量。结果两组病例治疗后均达到上前牙的有效内收和压低,U1-SN,U1-PP术后均减小(P<0.05),高位组减小量明显大(P<0.05),说明高位组的前牙内收转矩丢失相对较多。U6-PP术后均减小(P<0.05),高位组减小量显著大(P<0.05),说明高位组后牙压低效果相对较好。结论两法都能有效矫治牙性骨性混合型上颌前突。后牙低位植入加前牙区2颗联合牵引获得较多的上前牙整体内收,上颌骨改建明显。后牙高位植入获得较多的后牙压低,有效实现下颌骨的逆旋。

Objective To compare the clinical effects of two different positions of micro-screw implants on the treatment of dental and skeletal maxillary protrusion patients requiring extraction of premolar orthodontic therapy.Methods Thirty-eight patients with indications were selected.According to the implantable height of micro-screw implants in maxillary molar region,the patients were divided into high position group(20 cases)and low position group(18 cases).The changes of measurement indexes before and after treatment were analyzed statistically.Results The upper anterior teeth of all the patients got obvious retraction and intrusion.The data of U1-SN and U1-PP decreased significantly(P<0.05)after operation,while the High position group decreased more(P<0.05).The results showed that High position group lost more anterior teeth torque.The distance of U6-PP decreased(P<0.05)while High position group decreased significantly(P<0.05),indicating that posterior teeth were intruded,and High position group had better effects.Conclusion Both methods can be effective in the treatment of dental and skeletal maxillary protrusion.More retraction of the upper anterior teeth and the maxillary retraction can come out from Low position group.Better vertical control can be got from High position group,from which more intrusion of posterior teeth and more counter clockwise rotation of the mandible are obtained.
