探析深化医改后医院门诊量及收入结构的变化

A Brief Analysis of the Change of Outpatient Service Quantity and Income Structure After Deepening Medical Reform

目的落实分级诊疗制度,做好分级诊疗工作,改变门诊收入结构,改善就医环境。方法研究起止时间为2016年5月—2018年3月,对医改前医院门诊量及门诊收入结构进行分析,依据医改政策,采取加强宣传力度,做好分级诊疗,创建专科特色技术品牌,规范医生合理用药,改善就医环境等干预措施并统计。结果医院门诊量下降20.35%,门诊药费占比下降12.64%,门诊人均药费下降6.05%,门诊治疗费占比增加25.54%。经统计学分析,均显示P<0.05,差异具有统计学意义。结论依据医改落实分级诊疗制度及医疗费用总量控制可有效降低门诊人均药费,减轻患者医疗负担,医院管理中需引起重视。

Objective To implement the graded diagnosis and treatment system,do a good job in graded diagnosis and treatment,change the income structure of outpatient clinics and improve the medical environment.Methods The research started and ended from May 2016 to March 2018.The outpatient volume and income structure of our hospital before the medical reform were analyzed.According to the medical reform policy,interventions such as strengthening propaganda,doing a good job in grading diagnosis and treatment,creating special technical brands,standardizing doctors’rational drug use and improving the medical environment were taken.Results Outpatient service volume decreased by 20.35%,outpatient medicine cost decreased by 12.64%,outpatient per capita medicine cost decreased by 6.05%,outpatient treatment cost increased by 25.54%.Statistical analysis showed that all P<0.05,which indicated that there was statistical significance and difference between the two groups.Conclusion The implementation of graded diagnosis and treatment system and the total control of medical expenditure can effectively reduce the per capita outpatient drug costs and reduce the medical burden of patients,which should be paid attention to in hospital management.
