双歧杆菌三联活菌胶囊联合西咪替丁对早产儿坏死性小肠结肠炎降钙素原、C反应蛋白的影响

Effect of Bifidobacterium Triple Viable Capsule and Cimetidine on Necrotizing Enterocolitis and Procalcitonin and C-Reactive Protein in Preterm Infants

目的研究双歧杆菌三联活菌胶囊联合西咪替丁治疗坏死性小肠结肠炎(necrotizing enterocolitis,NEC)早产儿的疗效及对降钙素原(procalcitonin,PCT)和C反应蛋白(C-reactive protein,CRP)的影响。方法选择2017年6月—2019年12月我院收治的67例NEC早产儿随机分为观察组(34例)和对照组(33例),均接受常规治疗,对照组患儿在常规治疗基础上给予双歧杆菌三联活菌胶囊治疗,观察组患儿在对照组治疗基础上给予西咪替丁治疗,共治疗7 d。对比两组患者疗效,病程及体质量增长情况;对比两组患儿血清降钙素原(PCT)和C反应蛋白(CRP)水平。结果治疗后,观察组总有效率显著高于对照组(91.18%vs 63.64%,P<0.05);且观察组病程时间较对照组显著缩短[(4.1±1.1)d vs(6.2±1.2)d,P<0.05],患儿平均每日体质量较对照组显著增长[(14.5±3.2)g vs(8.3±1.8)g,P<0.05];观察组患儿PCT和CRP水平较治疗前和对照组均显著降低(P<0.05)。结论双歧杆菌三联活菌胶囊联合西咪替丁可有效改善NEC早产儿炎症状态,提高疗效。

Objective To study the efficacy of bifidobacterium triple viable capsule in combination with cimetidine in the treatment of premature infants with necrotizing enterocolitis(NEC)and its effect on procalcitonin(PCT)and C-reactive protein(CRP).Methods 67 cases of NEC premature babies in our hospital were randomly divided into observation group(34 cases)and control group(33 cases),all of whom received conventional treatment.The control group was given bifidobacterium triple viable capsule based on conventional treatment,and the observation group was given control.The group was treated with cimetidine for 7 days.The efficacy,course of disease and weight gain of the two groups were compared;the levels of serum procalcitonin and C-reactive protein were compared between the two groups.Results After treatment,the total effective rate in the observation group was significantly higher than that in the control group(91.18%vs 63.64%,P<0.05);and the duration of disease in the observation group was significantly shorter than that in the control group[(4.1±1.1)d vs(6.2±1.2)d,P<0.05],the average daily body weight of the children increased significantly compared with the control group[(14.5±3.2)g vs(8.3±1.8)g,P<0.05];PCT and CRP levels in the observation group were higher than before treatment.And the control group were significantly reduced(P<0.05).Conclusion Bifidobacterium triple viable capsule combined with cimetidine can effectively improve the inflammatory status and improve the efficacy of preterm infants with NEC.
