摘要
目的了解苏州地区皮肤性病门诊患者HPV感染情况及其基因亚型分布。方法采用PCR体外扩增和DNA反向点杂交相结合的基因检测技术,对1630例苏州地区皮肤性病门诊疑似HPV感染者进行25种HPV亚型检测,分析该人群HPV感染状况及HPV亚型流行情况。结果1630例疑似感染者中共检出HPV阳性903例,阳性检出率为55.39%,其中女性阳性检出率为60.31%(576/955),男性阳性检出率为48.44%(327/675),两者差异有统计学意义(χ^2=22.55,P<0.01)。女性感染率最高的高危亚型分别为HPV52、16、58、53和33,男性为HPV16、52、53、51和58。HPV感染女性高危亚型单一或混合感染率达77.08%,男性也达到58.41%。各年龄组女性HPV阳性检出率均超过55.00%,且以4~20岁和>50岁组检出率最高,分别达75.38%、71.70%,显著高于同龄男性组(χ^2=7.10、5.00,P<0.05);各年龄组男性HPV阳性检出率在40%~50%之间。结论苏州地区皮肤性病门诊患者HPV感染率较高,且以高危亚型感染为主,女性尤甚。
Objective To understand the HPV infection status and HPV subtypes among dermatological outpatients of Suzhou.Methods By using PCR and reverse dot blot genotyping assay kit,25 HPV subtypes were detected in 1630 suspected HPV infected outpatients of Suzhou to analyze the epidemic status and HPV subtypes.Results Among the 1630 suspected HPV infected patients,903 were positive with the positive rate of 55.39%.There was a significant difference between the positive rates in males(48.44%,327/675)and females(60.31%,576/955)(χ^2=22.55,P<0.01).High-risk subtypes with high infection rates were HPV52,16,58,53,33 in females and HPV16,52,53,51,58 in males.The single or mixed infection rates of high-risk HPV subtypes were 77.08%in females and 58.41%in males.The positive rates of HPV in all age groups of females exceeded 55.00%,and the highest rates were in 4-20 and>50 years old groups(75.38%and 71.70%,respectively),which were significantly higher than those in male groups(χ^2=7.10,5.00,P<0.05).The positive rates of HPV among different age groups of males were 40%-50%.Conclusions HPV infection rate in dermatological outpatients in Suzhou is high,especially in women with high-risk subtypes.
