Isolate密度梯度离心法和上游法对精子DNA碎片率及人工授精结局的影响 被引量:6

摘要 目的比较Isolate密度梯度离心法和上游法对精子DNA碎片指数(DFI)变化及宫腔内人工授精(IUI)结局的影响。方法328例行IUI的患者,按精子优化方法分为Isolate密度梯度离心法(n=164)和上游法(n=164)。精子优化前分为DFI≤20%组(n=200)和DFI>20%组(n=128)。分析两种精子优化前后精子DFI的变化及对IUI结局的影响。结果精子优化前Isolate密度梯度离心法和上游法之间的精子活力和精子DFI差异无统计学意义(P>0.05);处理后Isolate密度梯度离心法和上游法的精子活力均显著提高,且精子DFI显著降低(P<0.05)。处理后的精子活力,上游法显著高于Isolate密度梯度离心法(P<0.05)。处理后的精子DFI,Isolate密度梯度离心法和上游法在DFI≤20%中差异无统计学意义(P>0.05),但在DFI>20%中Isolate密度梯度离心法显著低于上游法(P<0.05)。IUI后的临床妊娠率,在DFI≤20%中的Isolate密度梯度离心法和上游法之间差异无统计学意义(P>0.05);在DFI>20%中Isolate密度梯度离心法显著高于上游法(P<0.05),变化趋势与处理后的精子DFI一致。结论Isolate密度梯度离心法处理精液后降低精子DNA碎片的效果优于上游法,且精子DFI可影响IUI的临床妊娠结局。
