摘要
目的:将两种基于转科的平均住院日计算方法与传统方法进行差异性比较,以寻找更加有效的计算方法,为医院管理提供支撑。方法:选取某三级甲等综合医院2018年12月份住院病例的住院首页和HIS数据信息,提取平均住院日和转科信息,运用已发表的两种基于转科平均住院日的计算方法分别进行计算和比较。结果:三种方法计算全院的平均住院日均相同,传统法、方法一和方法二期内收治病人数分别为10721、11232、10721人次;方法一计算的平均住院日大部分科室低于传统法,两者差别有统计学意义;方法二计算的平均住院日大部分科室也低于传统法,两者差别无统计学意义。并且两种方法与传统法均具有较强的相关性。结论:相比于方法一,运用分割法可以综合考虑转科与非转科的差别和影响,不会导致绝大多数科室的平均住院日均降低,是一种较为科学、全面的方法。
Objective:In order to find a more suitable method and provide support for hospital management,two methods of calculating average length of stay based on departments transference were compared with traditional methods.Methods:The information of average length of stay and transfer between departments in December 2018 were extracted from the home page and HIS of a tertiary hospital with level 3 in Jiangsu province.Two published calculation methods of average length of stay based on departments transference were used for calculation and comparison.Results:The three methods were used to calculate the same average length of stay of the whole hospital.The number of patients admitted in the traditional method,method 1 and method 2 were 10721,11232 and 10721,respectively.The average length of stay calculated by method 1 was lower than that by traditional method in most departments,and the difference was statistically significant.The average length of stay calculated by method 2 was also lower than the traditional method in most departments,and the difference was not statistically significant.And the two methods had strong correlation with the traditional method.Conclusion:The segmentation method can be used to comprehensively consider the difference and influence between transferred departments and non-transferred departments,and will not lead to the decrease of the average length of stay in most departments.It is a more scientific and comprehensive method compared with method 1.
