摘要
目的探讨P-选择素(PSEL)S290N及P-选择素糖蛋白配体-1(PSGL-1)M62I基因多态性与上海汉族人群颈动脉粥样硬化及急性脑梗死(acute cerebral infraction,ACI)的相关性。方法采用多重连接酶检测分型(multiple LDR)对上海地区197例颈动脉粥样硬化患者,182例ACI患者及对照组进行S290N及M62I基因多态性分析,收集3组临床资料并分析。结果颈动脉粥样硬化组年龄、吸烟史比例、高血压病比例、收缩压、空腹血糖水平、三酰甘油水平、载脂蛋白B水平明显高于对照组(P<0.05),PSGL-1基因型频率及等位基因频率与对照组相比差异有统计学意义(P<0.05)。ACI组吸烟史、高血压病史、糖尿病病史、收缩压及空腹血糖水平明显高于对照组(P<0.05),而高密度脂蛋白水平明显低于对照组(P<0.05),PSGL-1基因型频率及等位基因频率与对照组相比差异有统计学意义(P<0.05)。Logistic回归分析发现性别、年龄、吸烟史、高血压史、糖尿病病史、低密度脂蛋白水平及M62I基因型是颈动脉粥样硬化的独立危险因素。吸烟史、高血压、空腹血糖、高密度脂蛋白水平及M62I基因型是ACI的独立危险因素。结论PSELS290N基因多态性与上海汉族颈动脉粥样硬化及ACI无关。PSGL-1M62I基因多态性与上海汉族人群颈动脉粥样硬化及ACI有关,A等位基因突变可能是上海汉族人群颈动脉粥样硬化及ACI的保护性因素。
Objective To investigate the association of P-selectin(PSEL)S290 N gene and P-selectin glycoprotein ligand-1(PSGL-1)M62 I gene polymorphism with carotid atherosclerosis and acute cerebral infarction(ACI)in Shanghai Han population.Methods Multi-ligase detection(Multiple LDR)was used to analyze S290 N and M62 I gene polymorphisms in 197 patients with carotid atherosclerosis,182 patients with ACI and control group in Shanghai.Three groups of clinical data were collected and analyzed.Results The age,smoking history,history of hypertension,systolic blood pressure,fasting blood glucose level,triglyceride level and apolipoprotein B level in the carotid atherosclerosis group were significantly different from those in the control group(P<0.05),PSGL-1 genotype frequency and allele frequency were significantly different from the control group(P<0.05).Smoking history,history of hypertension,history of diabetes,systolic blood pressure,fasting blood glucose level and high-density lipoprotein level in the cerebral infarction group were significantly different from those in the control group(P<0.05),the frequency of PSGL-1 genotype and allele frequency were significantly different from the control group(P<0.05).Logistic regression analysis found that gender,age,smoking history,history of hypertension,history of diabetes,low-density lipoprotein level and M62 I genotype were independent risk factors for carotid atherosclerosis.Smoking history,history of hypertension,fasting blood glucose level,high-density lipoprotein level and M62 I genotype were independent risk factors for ACI.Conclusion The P-selectin S290 N gene polymorphism is not associated with carotid atherosclerosis or ACI in Shanghai Han population.P-selectin glycoprotein ligand-1 M62 I gene polymorphism is associated with carotid atherosclerosis and ACI in Shanghai Han population.A allele mutation may be a protective factor for carotid atherosclerosis and ACI in Shanghai Han population.
