25-(OH)D3和IL-17在不同严重程度哮喘患儿血清中的表达及其临床意义 被引量:6

摘要 目的探讨25-(OH)D3和白细胞介素-17(IL-17)在不同严重程度哮喘患儿血清中的表达及其临床意义。方法选取2016年10月-2018年12月海口市人民医院收治的哮喘患儿224例作为研究组,根据研究组患儿不同病情分为轻度间歇组68例、轻度持续组52例、中度持续组59例和重度持续组45例。选取该院同期健康体检儿童50例作为对照组。分别检测各组血清25-(OH)D3和IL-17水平;同时采用儿童哮喘控制测试评分系统(C-ACT)进行哮喘病情评分。比较各组血清25-(OH)D3和IL-17水平,分析血清25-(OH)D3、IL-17水平与C-ACT评分的相关性。结果观察组血清IL-17水平高于对照组(P<0.05),而血清25-(OH)D3水平低于对照组(P<0.05)。轻度间歇组、轻度持续组、中度持续组和重度持续组血清IL-17、25-(OH)D3水平和C-CAT评分比较,经方差分析,差异有统计学意义(P<0.05);从轻度间歇组、轻度持续组、中度持续组到重度持续组,血清IL-17水平呈上升趋势(P<0.05),而血清25-(OH)D3水平和C-CAT评分呈下降趋势(P<0.05)。Pearson相关性分析显示,观察组患儿血清IL-17与C-CAT评分呈负相关(r=-0.825,P=0.006),而血清25-(OH)D3水平与C-CAT评分呈正相关(r=0.917,P=0.008)。结论哮喘患儿血清25-(OH)D3水平降低,血清IL-17水平升高,且指标变化情况与病情有相关性。早期检测25-(OH)D3和IL-17水平,对哮喘早期诊断、病情及预后评估具有重要临床意义。 Objective To investigate the expression and clinical significance of 25-dihydroxy D3(25-(OH)D3)and interleukin-17(IL-17)in serum of children with asthma of different severity.Methods The subjects were 224 children with asthma admitted to our hospital from October 2016 to December 2018,and divided into four groups:mild intermittent group,mild persistent group,moderate persistent group and severe persistent group(68 cases,52 cases,59 cases and 45 cases,respectively).50 healthy children were selected as control group at the same time.Detection of serum 25-(OH)D3 and IL-1 in five groups of children at the same time,and the asthma condition score of four groups was measured by C-ACT(Child Asthma Control Test scoring system).The levels of serum 25-(OH)D3 and IL-17 in five groups were compared,and the correlation between serum 25-(OH)D3 and IL-17 levels and C-ACT score was also analyzed by Pearson correlation analysis.Results The serum level of IL-17 in the observation group was higher than that in the control group(P<0.05),while the serum level of 25-(OH)D3 was lower than that in the control group(P<0.05).The levels of serum IL-17,25-(OH)D3 and C-ACT scores in mild intermittent group,mild persistent group,moderate persistent group and severe persistent group were significantly different(P<0.05).From mild intermittent group,mild persistent group,moderate persistent group to severe persistent group,the serum IL-17 level showed an upward trend(P<0.05),while the serum 25-(OH)D3 level and C-ACT score showed a downward trend(P<0.05).Pearson correlation analysis showed that serum IL-17 was negatively correlated with C-ACT score(r=-0.825,P=0.006),while serum 25-(OH)D3 was positively correlated with C-ACT score(r=0.917,P=0.008).Conclusion The serum 25-(OH)D3 level significantly decreases and the serum IL-17 level increases in children with asthma,and there is a significant correlation between the changes of the two groups’indexes and the condition of the disease.The early detection of the levels of the two groups can be used in the early diagnosis of asthma.The evaluation of the condition and prognosis is of great clinical significance.
