血液透析联合血液灌流在慢性肾衰竭治疗中的应用价值体会

Application value of hemodialysis combined with hemoperfusion in the treatment of chronic renal failure

目的观察分析血液透析联合血液灌流在慢性肾衰竭治疗中的应用价值。方法选取本院2018年2月至2019年2月期间收治的44例慢性肾衰竭患者,按照数字随机表法分为实验组(22例,应用血液透析联合血液灌流治疗方法)和对照组(22例,单纯应用血液透析治疗方法)。采用统计学分析两组慢性肾衰竭患者治疗前后的血清瘦素水平、β2微球蛋白水平、糖化血红蛋白水平、促红细胞生成素水平以及健康调查表(社会功能、躯体疼痛、心理健康、精力、躯体角色功能以及总体健康状况)评分。结果治疗前,两组患者的血清瘦素水平、β2微球蛋白水平、糖化血红蛋白水平、促红细胞生成素水平比较,差异均无统计学意义(均P>0.05)。治疗后,实验组患者的血清瘦素水平、β2微球蛋白水平、糖化血红蛋白水平、促红细胞生成素水平分别为(5.7±2.6)mg/L、(18.8±6.5)mg/L、(132.6±3.7)g/L、(139.9±36.6)U/kg,对照组分别为(8.9±3.5)mg/L、(36.7±14.1)mg/L、(125.5±3.2)g/L、(128.8±32.4)U/kg,实验组慢性肾衰竭患者治疗后血清瘦素水平、β2微球蛋白水平均显著低于对照组(均P<0.05),糖化血红蛋白水平、促红细胞生成素水平显著高于对照组(均P<0.05)。实验组慢性肾衰竭患者的社会功能、躯体疼痛、心理健康、精力、躯体角色功能以及总体健康状况评分均显著高于对照组(均P<0.05)。结论血液透析联合血液灌流在慢性肾衰竭治疗中的应用价值显著。

Objective To observe and analyze the application value of hemodialysis combined with hemoperfusion in the treatment of chronic renal failure.Methods Forty-four patients with chronic renal failure in our hospital from February 2018 to February 2019 were selected and were divided into the experimental group(22 patients who were treated with hemodialysis combined with hemoperfusion)and the control group(22 patients who were treated with hemodialysis alone)according to the random number table.The levels of serum leptin,β2 microglobulin,hemoglobin a1c,erythropoietin,and the scores of health questionnaire(social function,physical pain,mental health,energy,somatic role function,and general health)before and after treatment in both groups of patients with chronic renal failure were statistically analyzed.Results Before the treatment,there were no statistically significant differences in the levels of serum leptin,β2 microglobulin,hemoglobin a1c,erythropoietin between the two groups(all P>0.05);after the treatment,the levels of serum leptin,β2 microglobulin,hemoglobin a1c,erythropoietin in the experimental group were(5.7±2.6)mg/L,(18.8±6.5)mg/L,(132.6±3.7)g/L,(139.9±36.6)U/kg,those in the control group were(8.9±3.5)mg/L,(36.7±14.1)mg/L,(125.5±3.2)g/L,(128.8±32.4)U/kg,the levels of serum leptin,β2 microglobulin in the experimental group were significantly lower than those in the control group(all P<0.05),the levels of hemoglobin a1c,erythropoietin in the experimental group were significantly higher than those in the control group(all P<0.05).After the treatment,the scores of social function,physical pain,mental health,energy,somatic role function,and general health of health questionnaire in the experimental group were significantly higher than those in the control group(all P<0.05).Conclusion Hemodialysis combined with hemoperfusion is of significant value in the treatment of chronic renal failure.
