2型糖尿病患者服药依从性应用个案管理模式的干预效果调查

Investigation on intervention effect of case management model for medication compliance in patients with type 2 diabetes mellitus

目的观察2型糖尿病患者采用个案管理模式对患者服药依从性的干预作用。方法选取2017年1月至2019年7月入住本院的2型糖尿病患者120例作为主要观察对象,采用随机数字表法将患者分为观察组(60例)、对照组(60例),对照组采用常规护理,观察组采用个案管理模式,比较两组患者的服药依从性、自我管理能力。结果观察组患者干预后6个月的用药依从性(高54例、中6例)均显著优于对照组同期及本组干预前(均P<0.05);观察组患者干预后6个月FPG(6.34±1.73)mmol/L、HbA1c(5.82±0.49)%、BMI(22.93±2.31)kg/m^2,均显著低于对照组同期及本组干预前(均P<0.05)。结论2型糖尿病患者服药依从性通过个案管理模式能够有效改善,且个案管理模式有助于降低患者的空腹血糖、糖化血红蛋白及体质量指数,具有一定的借鉴价值。

Objective To observe the intervention effect of case management model on medication compliance of patients with type 2 diabetes mellitus.Methods 120 cases of type 2 diabetes mellitus who were admitted to our hospital from January 2017 to July 2019 were selected as main observation objects.The patients were divided into the observation group(60 cases)and the control group(60 cases)by the random number table method.The control group received the routine nursing,and the observation group received the case management model.The medication compliance and self-management ability of the two groups were compared.Results The medication compliance of the patients in the observation group after nursing was significantly better than that of the control group(P<0.05).The level of FPG,HbA1c,BMI in the observation group after nursing[(6.34±1.73)mmol/L,(5.82±0.49)%,(22.93±2.31)kg/m2]were significantly lower than those in the control group at the same time point and those before nursing(P<0.05).Conclusion The medication compliance of type 2 diabetic patients can be effectively improved by case management model,and the case management model is helpful to reduce fasting blood glucose,glycosylated hemoglobin,and body mass index,which has certain reference value.
