巨大Pott膨胀瘤诊治并文献复习

Diagnosis and treatment of giant Pott’s puffy tumor and literature review

目的总结Pott膨胀瘤的临床表现、诊断及治疗方法,提高医务人员对Pott膨胀瘤的认识。方法对本院2016年3月收治的1例巨大Pott膨胀瘤的临床资料结合文献进行分析。结果患者,男57岁,因“额部渐增大肿物5年”入院。CT检查示额窦前壁及后壁骨质缺损,额部见约4.0 cm×2.5 cm×2.5 cm大小软组织包块,窦内软组织充填。行鼻外径路及鼻内镜联合路径额部肿物切除术。术后恢复良好,随访至今无临床症状。通过检索共检出中文文献2篇,英文文献共37篇。结论Pott膨胀瘤最初被描述为头部创伤的并发症,通常发生在额窦炎的并发症中,是额窦炎极为罕见的并发症。早期诊断、药物治疗和外科治疗对于降低发病率和病死率非常重要。

Objective To summarize the clinicopathological features and the diagnosis and treatment methods of Pott's puffy tumor,and to improve medical workers’understanding of Pott's puffy tumor.Methods The clinical data of 1 case of giant Pott's puffy tumor treated in our hospital in April,2016 and the literature data were analyzed.Results In this report,we describe the radiological findings of a 57-year-old man,presenting with swelling of the scalp.The patient underwent CT scan of the paranasal sinuses,which showed the following changes:erosion of anterior and posterior table of the frontal sinus,frontal sinus opaciication,and soft tissue swelling of the forehead and parietal region measuring 4.0 cm×2.5 cm×2.5 cm.The patient was opted for surgical drainage with combined approach,endoscopic endonasal and external drainage via coronal incision.The patient recovered well after the operation,and no new complaints showed in the follow-up so far.By literature review,we found 2 Chinese articles and 37 English.Conclusion Originally described as a complication of head trauma,Pott's puffy tumor typically occurs as a complication of frontal sinusitis.Pott's puffy tumor is an extremely rare complication of frontal sinusitis.Early diagnosis,medication,and surgical therapy are very important in reducing morbidity and mortality.
