自拟补肾通络方联合人工周期治疗卵巢储备功能下降所致不孕

Clinical Observation of Self-made Bushen Tongluo Decoction Combined with Artificial Cycle in the Treatment of Infertility Caused by Decreased Ovarian Reserve Function

目的:观察自拟补肾通络方联合"戊酸雌二醇+黄体酮"治疗卵巢储备功能下降所致不孕的临床疗效。方法:将我院符合肾虚血瘀、卵巢储备功能下降所致不孕60例患者分为治疗组和对照组,每组均30例。对照组采用"戊酸雌二醇+黄体酮"治疗,治疗组在对照组基础上加服补肾通络方治疗,1月为1疗程,共治疗3疗程,比较两组治疗后总疗效、治疗前后中医症候积分情况,治疗前后FSH、LH、AMH水平;随访3个月,比较两组妊娠率。结果:治疗组患者治疗总有效率为96.7%,对照组治疗总有效率的76.7%(P<0.05);治疗组治疗后月经提前、月经推迟、月经量少、烦躁焦虑、头晕耳鸣、失眠健忘、腰膝酸软和中医症候总积分低于对照组(P<0.05);治疗组治疗后FSH、LH水平低于对照组,治疗组治疗后AMH水平高于对照组(P<0.05);随访3个月,实验组、对照组妊娠率分别为40%、16.7%,治疗组均高于对照组。结论:补肾通络方联合人工周期比单独人工周期能更有效的降低患者的中医症候积分,降低FSH、LH水平,提高AMH水平,从而显著提高受孕机率。

Objective:To observe the clinical efficacy of self-made Bushen Tongluo Recipe combined with "estradiol valerate + progesterone" in the treatment of infertility caused by ovarian reserve decline.Methods:Sixty patients with infertility caused by kidney deficiency and blood stasis and decreased ovarian reserve function were divided into a treatment group and a control group,with 30 cases in each group.The control group was treated with "estradiol valerate + progesterone".The treatment group was treated with Bushen Tongluo Recipe on the basis of the control group.One month was a course of treatment,and a total of three courses were treated.TCM symptom score before and after,FSH,LH,AMH levels before and after treatment;follow-up for 3 months to compare the pregnancy rates between the two groups.Results:The total effective rate of treatment in the treatment group was 96.7%,and the total effective rate in the control group was 76.7% (P<0.05).In the treatment group,menstruation was advanced,menstruation was delayed,menstrual volume was low,irritability,anxiety,dizziness,tinnitus,insomnia and forgetfulness after treatment.The total scores of waist,knees,tenderness,and TCM symptoms were lower than those in the control group (P<0.05);FSH and LH levels in the treatment group were lower than those in the control group,and AMH levels were higher in the treatment group after treatment (P<0.05);Months,the pregnancy rates of the experimental group and the control group were 40% and 16.7%,respectively,and the treatment group was higher than the control group.Conclusion:The combination of Bushen Tongluo Recipe with artificial cycle can more effectively reduce the TCM symptom score,lower the levels of FSH and LH,and increase the level of AMH,thus significantly increasing the chance of conception.
