自由体位联合穴位按压减痛法对初产顺产产妇分娩效果及母婴情况的影响

Effect of Free Posture Combined with Acupoint Pressing and Pain Reduction method on Delivery Effect and Maternal and Infant Situation of Primipara and Spontaneous Delivery

目的:探讨自由体位联合减痛法在初产顺产产妇分娩中的应用效果及对母婴结局的影响。方法:选择2018年6月至2018年12月产科接收的90例待产初产孕妇,随机分为对照组和观察组,各45例。对照组采取自由体位分娩,观察组在自由体位基础上采取穴位按压减痛法,比较2组产妇产程、出血量、产时疼痛及焦虑情况,并比较两组新生儿Apgar评分。结果:对照组治疗后疼痛、焦虑评分均显著降低(P<0.01)。观察组产妇第二产程时间显著降低对照组(P<0.01),且胎心率异常率降低,但差异无统计学意义(P>0.05);产时出血量也显著降低(P<0.01)。2组中转剖宫产人数分别为6例、3例,无差异(P>0.05);而与对照组相比,观察组新生儿Apgar评分显著升高(P<0.01),婴儿安全性更优。结论:自由体位联合穴位按压减痛法可缩短产妇产程、降低出血量,母婴结局良好。

Objective:To investigate the effect of free position combined pain reduction method on the delivery of primiparous and parturient women and the effect on the outcome of mother and child.Methods:Ninety cases of primiparous pregnant women received from obstetrics from June 2018 to December 2018 were randomly divided into control group and observation group,with 45 cases in each group.The control group adopted free position labor,the observation group adopted acupressure reduction pain method based on the free position,compared the two groups of maternal labor,bleeding,pain and anxiety,and compared the Apgar score of the two groups of newborns.Results:The pain and anxiety scores of the control group were significantly reduced after treatment (P<0.01).In the observation group,the time of the second stage of labor was significantly reduced in the control group (P<0.01),and the abnormal rate of fetal heart rate was reduced,but the difference was not statistically significant (P>0.05).The amount of bleeding during delivery was also significantly reduced (P<0.01).The number of cesarean delivery patients in the two groups was 6 and 3,respectively,and there was no difference (P>0.05).Compared with the control group,the Apgar score of the newborns in the observation group was significantly increased (P<0.01),and the infants had better safety.Conclusion:Free posture combined with acupressure reduction can shorten the process of maternal labor and reduce the amount of bleeding,and the outcome of mother and infant is good.
