结肠镜检查采用节段重复退镜观察的研究

Study on examination with repeated segmental withdrawal during colonoscopy

目的探讨节段重复退镜观察是否可提高息肉的检查率。方法纳入2018年7月~2019年7月在广安市人民医院内镜中心行结肠镜检查的400例患者,采用Excel随机数字法分为节段重复退镜组及常规退镜组,每组200例,统计两组息肉的检出率。结果 400例结肠镜检查患者中,检出结直肠息肉127例(31.75%)其中节段重复退镜组息肉检出率为70例(35.00%),常规退镜组息肉检出率为57例(28.50%)(P> 0.05);节段重复退镜组与常规退镜组平均检出息肉数为(1.05±2.16)枚和(0.62±1.57)枚(P <0.05)。两组中男性患者息肉检出率均高于女性患者,差异显著(P <0.05);男性患者检出息肉数为(0.83±1.76)枚、女性患者为(0.34±0.83)枚(P <0.05)。节段重复退镜组息肉漏诊率为6.08%,常规退镜组息肉漏诊率为18.81%(P <0.05)。结论节段性重复退镜可有效提高结肠息肉的检出率,降低息肉的漏诊率。

Objective To explore whether examination with repeated segmental withdrawal can increase the detection rate of polyps.MeAods A total of 400 patients who received colonoscopy at Endoscopy Center of West China-Guang'an Hospital,Sichuan University from July 2018 to July 2019 were selected and divided into group A(examination with repeated segmental withdrawal,n=200)and group B(examination with conventional withdrawal,n=200)according to the random number table generated by Excel.The detection rates of polyps in the two groups were statistically analyzed.Results A total of 127 colon polyps were detected among 400 patients undergoing colonoscopy,with the detection rate of 31.75%.The detection rates of polyps in group A and group B were 35.00%(n=70)and 28.50%(n=57),respectively(P>0.05).The number of polyps detected in group A and group B was(1.05±2.16)mei and(0.62±1.57)mei,respectively(P<0.05).In both groups,the detection rate of polyps in male patients was higher than that in female patients,with significant difference(P<0.05).The number of polyps detected in male patients and female patients was(0.83±1.76)mei and(0.34±0.83)mei,respectively(P<0.05).The miss rates of polyps in group A and group B were 6.08%and 18.81%(P<0.05),respectively.Conclusion Repeated segmental withdrawal can effectively increase the detection rate of colon polyps and lower the miss rate.
