右半结肠侧向发育性肿瘤内镜黏膜下剥离术预后的分析

Analysis on prognosis of ESD for LSTs in right colon

目的分析右半结肠侧向发育性肿瘤患者行内镜黏膜下剥离术后,其预后的影响因素。方法选择2017年7月~2019年7月于医院接受内镜黏膜下剥离手术的161例右半结肠侧向发育性肿瘤患者为研究对象,依据预后是否良好分成良好组(124例)和欠佳组(37例)。以单因素分析、多因素分析确定患者手术预后的影响因素。结果两组右半结肠侧向发育性肿瘤患者在病灶大小、病灶位置、手术时间、肌层损伤、术中是否伴严重出血等比较差异显著(P <0.05);右半结肠侧向发育性肿瘤患者行内镜黏膜下剥离手术后,其预后的影响因素包含病灶大小、病灶位置、手术时间、肌层损伤、术中严重出血。结论右半结肠侧向发育性肿瘤患者行内镜下黏膜剥离手术后,其预后的影响因素较多,应结合影响因素类型,加强对右半结肠侧向发育性肿瘤患者的围手术期管理,弱化相关不利因素的影响,以改善其预后。

Objective To analyze the factors afecting the prognosis of endoscopic submucosal disection(ESD)for laterally spreading tumors(LSTs)in the right colon.Methods A total of 161 patients with LSTs in the right colon who underwent ESD in our hospital between July 2017 and July 2019 were selected as subjects of the study and divided into group A(good prognosis,n=124)and group B(poor prognosis,n=37)according to the condition of prognosis.Univariate and multivariate analyses were used to determine the factors affecting the prognosis of patients undergoing the procedure.Results There were significant differences between the two groups in lesion size and location,time of operation,muscular layer injury,and presence of severe bleeding during the operation(P<0.05).The factors afcting the prognosis of patients with LSTs in right colon undergoing ESD included lesion size and location,time of operation,muscular layer injury,and severe bleeding during the operation.Conclusion There are many factors affecting the prognosis of patients with LSTs in right colon undergoing ESD.To improve the prognosis,the type of affecting factors should be based on to strengthen the perioperative management of patients with LSTs in the right colon and attenuate the influence of relevant adverse factors.
