幽门螺杆菌和外周血中嗜酸性粒细胞与冠心病相关性分析 被引量:3

摘要 目的探讨幽门螺杆菌(Hp)感染和外周血中嗜酸性粒细胞(EOS)与冠状动脉粥样硬化性心脏病(CAD)的相关性。方法根据冠脉造影(CAG)的结果,将520例患者分成对照组257例和冠心病组263例。冠心病组根据血管病变的支数再分为:单支冠脉病变组患者95例、双支冠脉病变组患者80例、多支冠脉病变组患者88例。所有病例均行尿素13C-呼气试验、血浆Hp抗体测定及血常规中EOS计数水平,并计算冠心病组Gensini积分;同时收集白细胞计数、血糖水平、高敏C反应蛋白、高同型半胱氨酸等指标,评价Hp感染值、EOS水平与冠心病的相关性。结果①两组患者吸烟、年龄、低密度脂蛋白、高敏C反应蛋白、高同型半胱氨酸比较有统计学差异(P<0.05);②冠心病组Hp感染值、Gensini积分明显高于对照组(P<0.05),EOS水平低于对照组(P<0.05);③病变血管数增加,Hp感染值及Gensini积分也随着升高(P<0.05),而EOS水平是降低的(P<0.05);④Hp感染值与Gensini积分呈正相关(r=0.891,P<0.01),EOS水平与Gensini积分有相关性(r=-0.723,P<0.01),Hp感染值与EOS呈负相关(r=-0.826,P<0.01)。结论Hp感染值和EOS水平对冠心病有一定的预测价值。 Objective To investigate the correlation among infection of Helicobacter pylori(Hp),peripheral eosinophil(EOS)and coronary artery disease(CAD).Methods All patients(n=520)were divided,according to the results of coronary angiography(CAG),into control group(n=257)and CAD group(n=263).CAD group was divided further,according to lesion vessel number,into 1-vessel lesion group(n=95),2-vessel lesion group(n=80)and multi-vessel lesion group(n=88).The examination of 13C-urea breath test(13C-UBT)was carried out,levels of plasma Hp antibody and EOS count were detected in all patients,and Gensini integrals were calculated in CAD group.Meanwhile the levels of white blood cell count(WBC),blood sugar,high-sensitivity C-reactive protein(hs-CRP)and hyperhomocysteine(HCY)were detected.The correlation among Hp infection value,EOS level and CAD was reviewed.Results①The comparison in smoking,age,low-density lipoprotein(LDL),hs-CRP and HCY showed statistical difference between control group and CAD group(P<0.05).②Hp infection value and Gensini integrals were significantly higher(P<0.05)and EOS level was lower(P<0.05)in CAD group than those in control group.③As the increase of lesion vessel number,Hp infection value and Gensini integrals increased(P<0.05),and EOS level decreased.④Hp infection value was positively correlated to Gensini integrals(r=0.891,P<0.01),EOS level was correlated to Gensini integrals(r=-0.723,P<0.01),and Hp infection value was negatively correlated to EOS level(r=-0.826,P<0.01).Conclusion Hp infection value and EOS level have some predictive value to CAD.
