超重/肥胖与弥漫大B细胞淋巴瘤患者预后相关性的Meta分析

Correlation between overweight/obesity and prognosis in patients with diffuse large B-cell lymphoma:a Meta-analysis

目的系统评价超重/肥胖与弥漫大B细胞淋巴瘤(DLBCL)患者预后的关系。方法计算机检索PubMed、Embase、Cochrane Library、中国知网和万方等数据库,搜集关于体质指数与DLBCL预后相关性的队列研究。提取相关资料后,采用RevMan 5.3软件进行Meta分析。结果共纳入9篇文献。Meta分析结果显示,超重或肥胖DLBCL患者与正常体重DLBCL患者的总生存时间差异均无统计学意义(均P>0.05)。亚组分析结果显示,在北美洲亚组及以WHO标准定义亚组的患者中,超重DLBCL患者的总生存时间长于正常体重患者(均P<0.05);在欧洲亚组以及以非WHO标准定义亚组中,肥胖DLBCL患者的总生存时间短于正常体重患者(均P<0.05)。结论超重及肥胖可能与DLBCL患者的预后有关,其可能受种族及体质指数分类标准的影响。

Objective To systematically evaluate the relationship between overweight/obesity and prognosis in patients with diffuse large B-cell lymphoma(DLBCL). Methods A computer retrieval of PubMed, Embase, Cochrane Library, CNKI, Wangfang and other databases was conducted to collect the cohort studies on correlation between body mass index and prognosis in DLBCL. After extracting the relevant data, Meta-analysis was performed using RevMan 5.3 software. Results Nine articles were included totally. Meta-analysis results showed that there was no statistically significant difference in overall survival time between DLBCL patients with overweight/obesity and DLBCL patients with normal weight( all P>0.05). The results of subgroup analysis showed that, in patients of the North American subgroup and the subgroups defined in accordance with the WHO criteria, overweight DLBCL patients had longer overall survival time than those with normal weight(all P<0.05);in the European subgroup and the subgroups defined not using the WHO criteria, obese DLBCL patients exhibited shorter overall survival time than those with normal weight(all P<0.05). Conclusion Overweight and obesity may be related to the prognosis of DLBCL patients, which may be affected by race and body mass index classification.
