摘要
目的探讨Orem自理理论对胃癌患者术后癌因性疲乏(CRF)及舒适的影响。方法依据干预方式的不同将124例行胃癌根治术的胃癌患者分为对照组(n=68)和干预组(n=56),分别给予常规干预和Orem自理理论干预。干预15天后,比较两组患者的疲乏程度、自护能力、舒适度和生活质量。结果干预后,干预组患者躯体、情感和认知疲乏的评分均低于对照组,差异均有统计学意义(P﹤0.05)。干预后,干预组患者自我概念、自我责任感、自我护理技能和健康知识水平的评分和简化舒适状况量表(GCQ)各维度评分均高于对照组,差异均有统计学意义(P﹤0.05)。干预后,干预组患者吞咽困难、胃部疼痛、焦虑、饮食受限、呃逆和口干的评分均低于对照组,差异均有统计学意义(P﹤0.05)。结论Orem自理理论能够改善胃癌患者术后出现的CRF及相关临床症状,促进患者舒适,改善患者生活质量。
Objective To explore the effect of Orem’s self-care theory on postoperative cancer-related fatigue(CRF)and comfort in patients with gastric cancer.Method A total of 124 patients who underwent elective radical gastrectomy were divided as per different interventions into control group(n=68)and intervention group(n=56),which were administered with conventional interventions and specific interventions based on Orem’s self-care theory.After 15 days of intervention,degree of fatigue,self-care ability,comfort and quality of life in the two groups were compared.Result After intervention,the physical,emotional and cognitive fatigue scores in intervention group were significantly lower than those in control group(P<0.05),with evidently higher scores with respect to self-concept,self-responsibility,self-care skills and health knowledge as well as each dimension of general comfort questionnaire(GCQ)scale in intervention group versus control group,and the differences were of statistical significance(P<0.05).In the intervention group,the scores of dysphagia,stomach pain,anxiety,restricted diet,hiccup and dry mouth were decreased compared to control group(P<0.05).Conclusion Orem’s self-care theory has a significant effect on improving the CRF and associated clinical symptoms of patients with gastric cancer after surgery,promoting comfort and improving the quality of life of patients.
