腹腔镜联合米非司酮对卵巢巧克力囊肿的治疗效果评价

Evaluation of Laparoscopic Combined with Mifepristone in the Treatment of Ovarian Chocolate Cyst

目的探讨腹腔镜联合米非司酮对卵巢巧克力囊肿的治疗效果。方法选取卵巢巧克力囊肿患者80例,按照随机数字表法,分为对照组与观察组,各40例。两组均接受腹腔镜下巧克力囊肿剥除术治疗,观察组在对照组基础上加用米非司酮口服治疗,比较两组临床疗效、治疗前后卵巢功能指标以及复发情况。结果观察组治疗总有效率高于对照组,差异具有统计学意义(P<0.05);观察组术后1年复发率低于对照组,差异具有统计学意义(P<0.05);两组治疗前后血清促卵泡激素(FSH)、雌二醇(E2)、促黄体生成素(LH)与孕酮(P)水平差异均无统计学意义(P>0.05);观察组用药过程中发生头痛3例,胃部不适1例,肝功能异常3例,经对症处理后不良反应均消除。结论腹腔镜联合米非司酮可有效提升卵巢巧克力囊肿治疗效果,降低复发率,且不会明显影响患者机体卵巢功能。

Objective To investigate the effect of laparoscopy combined with mifepristone in the treatment of ovarian chocolate cyst.Methods 80 cases of chocolate cyst of ovary were selected and divided into control group and observation group with 40 cases each.Both groups were treated with laparoscopic chocolate cyst stripping,and the observation group was treated with oral mifepristone on the basis of the control group.The clinical efficacy,ovarian function indicators and recurrence before and after treatment were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).The recurrence rate of the observation group was lower than that of the control group one year after surgery,with statistically significant difference(P<0.05).There was no significant difference in serum folliclestimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH)and progesterone(P)between the two groups before and after treatment(P>0.05).In the observation group,there were 3 cases of headache,1 case of stomach discomfort,and 3 cases of abnormal liver function,all of which were eliminated after symptomatic treatment.Conclusion Laparoscopy combined with mifepristone can effectively improve the treatment effect of ovarian chocolate cyst,reduce the recurrence rate,and will not significantly affect the ovarian function of patients.
