米非司酮联合米索前列醇治疗瘢痕妊娠的临床分析

Clinical Analysis of Mifepristone Combined with Misoprostol in the Treatment of Scar Pregnancy

目的研究米非司酮联合米索前列醇治疗瘢痕妊娠的临床效果。方法随机抽取120例瘢痕妊娠患者,采用均衡分组法分为参照组和联合组,各60例。参照组采用米非司酮治疗,联合组采用米非司酮联合米索前列醇治疗。观察治疗效果,包括治疗总有效率、不良反应发生率、子宫出血量、子宫大小、孕囊直径大小及治疗前后血清β-HCG指标变化。结果联合组治疗总有效率95.00%,参照组治疗总有效率75.00%,对比有统计学意义(P<0.05)。联合组不良反应发生率3.33%,参照组不良反应发生率23.33%,对比有统计学意义(P<0.05)。联合组子宫出血量与参照组相比要少,子宫大小及孕囊直径与参照组相比要小,对比有统计学意义(P<0.05)。治疗前,组间血清β-HCG指标对比无差异,无统计学意义(P>0.05)。治疗后,联合组血清β-HCG指标与参照组及治疗前相比要高,对比有统计学意义(P<0.05)。结论米非司酮联合米索前列醇治疗瘢痕妊娠的临床效果显著,可减少不良反应发生,使其子宫、孕囊状态及血清β-HCG指标得到改善,值得应用于临床推广。

Objective To study the clinical effect of mifepristone combined with misoprostol in the treatment of scar pregnancy.Methods A total of 120 patients with scar pregnancy were randomly selected and divided into a reference group and a combined group by 60 groups.The reference group was treated with mifepristone and the combination group was treated with mifepristone plus misoprostol.The therapeutic effects were observed,including the total effective rate of treatment,the incidence of adverse reactions,the amount of uterine bleeding,the size of the uterus,the size of the gestational sac and the changes of serumβ-HCG levels before and after treatment.Results The total effective rate of the observation group was 95.00%,and the total effective rate of the control group was 75.00%.The correlation was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 3.33%,and the incidence of adverse reactions in the control group was 23.33%.The correlation was statistically significant(P<0.05).The amount of uterine bleeding in the observation group was less than that in the control group.The size of the uterus and the diameter of the gestational sac were smaller than those of the control group,and the correlation was statistically significant(P<0.05).There was no difference in serumβ-HCG between the groups before treatment,and there was no statistical significance(P>0.05).After treatment,the serumβ-HCG index of the observation group was higher than that of the control group and before treatment,and the correlation was statistically significant(P<0.05).Conclusion The clinical effect of mifepristone combined with misoprostol in the treatment of scar pregnancy is significant,which can reduce the occurrence of adverse reactions,improve the uterus,gestational sac state and serumβ-HCG index,which is worthy of clinical application.
