高血压性脑出血早期甘露醇不同剂量的降颅压效果差异分析

Analysis of the Difference in the Effect of Reducing the Intracranial Pressure with Different Doses of Mannitol in the Early Stage of Hypertensive Cerebral Hemorrhage

目的分析对比早期采用不同剂量甘露醇治疗高血压性脑出血患者的降颅压效果。方法选取2017年6月至2018年5月我院诊治的部分高血压性脑出血患者作为本次研究对象,共67例,随机分成两组,对照组33例,首先给予患者速尿20 mg,进行一次性静脉注射,然后在6 h之后使用250 mL(常规剂量)甘露醇治疗,观察组34例,在发病24 h之内给予患者125 mL甘露醇治疗,每天6~12 h静脉注射一次,患者发病2 d后复查头颅CT,观察治疗效果;对两组患者的生活能力进行评价,观察组明显优于对照组(P<0.05),有统计学意义。结果对照组有效率为81.82%,观察组有效率为85.29%,两组对比差异无统计学意义(P>0.05);两组在出血量以及神经功能评分方面比较,差异无统计学意义(P>0.05)。结论对于高血压性脑出血患者来说,早期使用半常规剂量甘露醇进行治疗,同样也可以取得较好的降颅内压效果,值得临床推广和应用。

Objective To analyze the effect of early reduction of intracranial pressure in patients with hypertensive intracerebral hemorrhage treated with different doses of mannitol.Methods A total of 67 patients with hypertensive intracerebral hemorrhage who were diagnosed and treated in our hospital from June to May 2018 were enrolled in this study.They were randomly divided into two groups,33 patients in the control group,and the first patient was given furosemide 20 mg.One-time intravenous injection,and then treated with 250 mL(conventional dose)mannitol after 6 h,34 patients in the observation group,125 mL mannitol treatment within 24 h of the onset,intravenous injection 6-12 h every day,after 2 d onset Review the head CT and observe the treatment effect.Results The effective rate of the control group was 81.82%,and the effective rate of the observation group was 85.29%.There was no significant difference between the two groups(P>0.05).There was no significant difference in the amount of bleeding and neurological function between the two groups(P>0.05);The vitality of the two groups of patients was evaluated.The observation group was significantly better than the control group(P<0.05),which was statistically significant.Conclusion For patients with hypertensive intracerebral hemorrhage,early treatment with semi-conventional doses of mannitol can also achieve better intracranial pressure reduction,which is worthy of clinical promotion and application.
