3000例青春期门诊妇科疾病患者疾病特征的调查分析

Investigation and analysis on the diseases characteristics of 3 000 adolescent gynecological outpatients

目的分析青春期门诊妇科疾病的患病情况,为青少年女性疾病防治提供科学的指导依据。方法选取2017年11月到2018年10月昆明市妇幼保健院青春期门诊就诊患者3 000例,分析其妇科疾病患病情况。结果①在3 000例青春期妇科疾病患者中,病种在前5位的依次是:青春期功能失调性子宫出血(青春期功血)(30.2%)、意外妊娠(21.4%)、多囊卵巢综合征(20.8%)、生殖道感染(16.0%)及痛经(7.0%)。②多囊卵巢综合征及青春期功血的检出率比较,在校组明显高于非在校组,差异具有统计学意义(P<0.05);意外妊娠、异位妊娠及性传播疾病的检出率比较,非在校组明显高于在校组,差异具有统计学意义(P<0.05)。③有性生活组多囊卵巢综合征及生殖道感染的检出率明显高于无性生活组,差异具有统计学意义(P<0.05)。结论青春期功血、意外妊娠、多囊卵巢综合征、生殖道感染及痛经是青春期女性常见妇科疾病,临床应根据青春期妇科疾病的发病特点,为这一群体提供专业的服务。

Objective To analyze the disease characteristics of adolescent gynecological outpatients, in order to provide basis for the prevention and treatment of adolescent gynecological diseases. Methods 3 000 adolescents with gynecological diseases in Kunming Woman and Child Health Hospital from November 2017 to October 2018 were selected to analyze the prevalence of gynecological diseases. Results ① Among the 3 000 adolescents, the top five diseases with the highest morbidity were puberty dysfunctional uterine bleeding(30.2%), unexpected pregnancy(21.4%), polycystic ovary syndrome(20.8%), genital tract infection(16.0%) and dysmenorrhea(7.0%). ② The detection rate of polycystic ovary syndrome and puberty dysfunctional uterine bleeding in the school group were obviously higher than those in the non-school group, with statistically significant differences(P<0.05);The detection rates of unexpected pregnancy, ectopic pregnancy and sexually transmitted diseases in the non-school group were obviously higher than those in the school group, with statistically significant differences(P<0.05). ③ The detection rates of polycystic ovary syndrome and genital tract infection in the sexual life group were obviously higher than those in the non-sexual life group, with statistically significant differences(P<0.05). Conclusions Puberty dysfunctional uterine bleeding, unexpected pregnancy, polycystic ovary syndrome, genital tract infection and dysmenorrhea are most common adolescent gynecological diseases, and professional clinical services should be provided according to characteristics of adolescent gynecological diseases.
