宁夏眼科医院患者术前血清梅毒抗体检测结果分析

Analysis on the Preoperative Testing Results of Treponema Pallidum Antibody in Ningxia Ophthalmology Hospital

目的分析宁夏眼科医院患者术前梅毒螺旋体抗体(TP-Ab)检测结果,了解TP-Ab阳性检出情况及分布特征,为临床预防和治疗提供依据。方法采用化学发光微粒子免疫检测法(CMIA)对2017年3月-2018年9月就诊于宁夏眼科医院的手术患者进行术前TP-Ab初筛,阳性者进行梅毒螺旋体明胶颗粒凝集试验(TPPA)确认和梅毒苯甲胺红不加热血清学(TRUST)检测。结果9733例患者术前检出TP-Ab阳性203例,男性总阳性检出率为2.33%,女性为1.85%,差异无统计学意义(P>0.05);20~39岁男性TP-Ab阳性检出率高于女性(P<0.05)。五个年龄段TP-Ab阳性检出率分别为0.00%、1.31%、1.65%、3.28%和3.53%,差异有统计学意义(P<0.05)。TP-Ab阳性患者中,血清TRUST效价阳性检出率男性(47.7%)低于女性(65.2%)(P<0.05);四个年龄段(除<20岁外)TP-Ab阳性患者中TRUST<1:8者构成比分别为62.5%、84.1%、95.9%和95.2%,差异有统计学意义(P<0.05)。结论眼病手术患者的梅毒血清学检测结果有年龄和性别差异,老年群体应引起重视。对TP-Ab阳性且血清TRUST阴性或低效价患者应长期随访,以控制梅毒的传播。

Objective By analyzing the preoperative testing results of Treponema pallidum antibody in patients of Ningxia Ophthalmology Hospital so as to understand the positive rate and the distribution of TP-Ab and to provide the basis for the clinical prevention and control of syphilis.Methods The measure called chemiluminescent immunoassay(CMIA)was taken in order to measure treponema pallidum antibody of preoperative patients from Mar.2017 to Sep.2018 in Ningxia Ophthalmology Hospital.Then the positive samples were determined by Treponema pallidum particle agglutination test(TPPA)and were tested by syphilis toluidine red unheated serum regain test(TRUST).Results Totally 203 positive cases of TP-Ab were confirmed in 9733 preoperatve patients.For male and female patients,the positive rates of TP-Ab were 2.33%and 1.85%without statistically significant difference of(P>0.05).However,the positive rates of TP-Ab in male and female patients at 20-39 years old were statistically significant(P<0.05).The positive rates of five groups were0.00%、1.31%、1.65%、3.28%、3.53%,with statistically significance(P<0.05).The percentage of TRUST positive was 47.7%in males and 65.2%females,with statistical significance(P<0.05).The TP-Ab positive patients with TRUST<1:8 accounted for 62.5%、84.1%、95.9%、95.2%in four groups(except for<20),with statistically significance(P<0.05).Conclusion The results of syphilis serology tests in gender and age have their own characteristics.The prevention and controlling measures for syphilis should be strengthened,especially in the senior population and long-term follow up of patients with TPPA positive and TRUST negative so as to controll the spread of syphilis.
