冠心病血瘀证与血管内皮功能及炎性因子的相关性研究 被引量:24

摘要 目的探究冠心病血瘀证与血管内皮功能及炎性因子的相关性。方法纳入2018年1月—2018年7月于中日友好医院中西医结合心内科住院治疗的冠心病病人180例(研究组)和55例非冠心病非血瘀证病人(对照组),收集病人中医四诊信息,检测血流介导的肱动脉血管舒张功能(FMD)、超敏C反应蛋白(hs-CRP)和生化指标,并进行冠心病血瘀证评分,根据积分高低分为高血瘀证积分组(56例)、低血瘀证积分组(62例)、非血瘀证组(62例)。结果FMD呈高血瘀证积分组<低血瘀证积分组<非血瘀证组<对照组递增的趋势(P<0.05);hs-CRP呈高血瘀证积分组>低血瘀证积分组>非血瘀证组>对照组递减趋势(P<0.05)。单因素线性回归分析结果显示FMD、hs-CRP与冠心病血瘀证积分相关(P<0.05);进一步多因素线性回归分析结果显示FMD[OR=-1.396,95%CI(-2.103,-0.689),P=0.000]、hs-CRP[OR=0.732,95%CI(0.460,1.003),P=0.004]与冠心病血瘀证积分依然存在相关性。结论血管内皮功能失调、机体炎症反应是冠心病病人血瘀证形成的病理学机制之一。 Objective To investigate the association of blood stasis syndrome with vascular endothelial function and inflammatory factors in coronary heart disease(CHD).Methods A total of 180 CHD patients(study group)and 55 patients with non-CHD and blood stasis syndrome who were not treated with CHD(control group)were enrolled.Data,blood flow-mediated brachial vasodilation(FMD),high-sensitivity C-reactive protein(hs-CRP)and biochemical indicators were measured.The blood stasis syndrome scores in CHD were recorded.According to the scores,the patients were divided into high blood stasis scores group(n=56),low blood stasis syndrome score group(n=62),and non-blood stasis syndrome group(n=62).Results FMD showed a increasing trendaney in high blood stasis syndrome score group,low blood stasis syndrome score group,non-blood stasis syndrome group,control group(P<0.05).Hs-CRP showed a decreasing trend aney in high blood stasis syndrome score group,low blood stasis syndrome score group,non-blood stasis group,and control group(P<0.05).Univariate linear regression analysis showed that FMD and hs-CRP were associated with CHD blood stasis syndrome scores(P<0.05).Further multivariate linear regression analysis showed FMD[r=-1.396,95%CI(-2.103,-0.689),P=0.000],and hs-CRP[OR=0.732,95%CI(0.460,1.003),P=0.004]is related to CHD blood stasis syndrome.Conclusion Vascular endothelial dysfunction and inflammatory response are the pathological mechanisms of blood stasis syndrome in patients with CHD.
