摘要
目的观察超声离子导入秋水仙碱治疗急性痛风性关节炎(AGA)的临床疗效以及对其相关炎性因子血清IL-1β、TNF-α、COX-2水平的影响。方法选取120例经临床确诊的急性痛风性关节炎患者随机平均分为治疗组和对照组,其中治疗组给予药物超声离子导入,对照组给予秋水仙碱口服治疗,两组均连续治疗14d后进行疗效评价。治疗前后抽取晨起空腹静脉血,离心提取血清,分别测定肿瘤坏死因子-α(TNF-α)、环氧化酶-2(COX-2)、白细胞介素-1β(IL-1β)。结果超声离子导入药物治疗急性痛风性关节炎具有较好的疗效,治疗两个疗程后,治疗组总有效率为91.67%,对照组93.33%;两组总有效率比较及总体疗效比较,差异均无统计学意义(p>0.05)。两组患者的肿瘤坏死因子-α(TNF-α)、环氧化酶-2(COX-2)、白细胞介素-1β(IL-1β)指标较治疗前下降,差异有统计学意义(p<0.05),经过两个疗程的治疗后,两组患者炎性指标相比较明显改善(p<0.01)。结论超声离子导入秋水仙碱对AGA有显著疗效,其疗效机制可能是通过抑制患者机体炎性反应而阻断或减轻关节局部炎性反应,从而抑制病程的发展,取得满意疗效。
Objective To observe the clinical efficacy of ultrasound iontoprotection of colchicine in the treatment of acute gouty arthritis and the effect on serum IL-1,TNF and COX-2 levels of its related inflammatory factors.Methods 120 patients diagnosed with acute gouty arthritis were randomly divided into treatment group and control group,60 patients in each group.The treatment group was given ultrasound iontophoresis,and the control group was given colchicine orally.The efficacy of both groups was evaluated after 14 days of continuous treatment.Fasting venous blood was extracted before and after treatment,and serum was centrifuged to determine tumor necrosis factor-a(TNF-a),cyclooxygenase-2(COX-2),and interleukin-1(IL-1β).Results Ultrasound iontophoresis had a good effect on the treatment of acute gouty arthritis.After two courses of treatment,the total effective rate of the treatment group was 91.67%,and that of the control group was 93.33%.There were no significant differences in total effective rate and overall efficacy between the two groups(p>0.05).Tumor necrosis factor-alpha(TNF-a),cyclooxygenase-2(COX-2),and interleukin-1(IL-1β)of the two groups were significantly lower than before treatment(p<0.05),and the inflammatory indexes of the two groups were significantly improved after two courses of treatment(p<0.01).Conclusions Ultrasound iontophoresis therapy for acute gouty arthritis had good efficacy,it could effectively improve the clinical symptoms and pain score of patients,safely shorten the course of disease with no side effects.
