远程延伸护理模式对慢性肾脏疾病患者治疗依从性、透析充分性和生活质量的影响

Effect of remote extended nursing model on treatment compliance, dialysis adequacy,and life quality in patients with chronic kidney disease

目的:研究以护士为主导的远程延伸护理模式对患有慢性肾脏疾病的透析患者治疗依从性、透析充分性和生活质量的影响。方法:选择2018年1月至2018年12月就诊于河北省保定市第一医院被诊断为慢性肾衰竭且进行规律透析治疗的患者共148例,按随机数字表法分为常规延伸护理组和基于微信远程干预组,每组74例,两组均予常规药物及规律透析治疗,常规延伸护理组给予常规护理,基于微信远程干预组给予常规护理基础上联合远程延伸护理,对所有研究对象进行临床检查、化验及问卷调查,比较两组患者治疗依从性、透析充分性、生活质量、护理满意度及透析并发症发生情况。结果:干预后,基于微信远程干预组患者的透析方案、饮食依从性、URR及KT/V、社会功能、生理功能、精神状态、总体健康评分和护理满意度显著高于常规延伸护理组(P<0.05);透析并发症发生率显著低于常规延伸护理组(P<0.05)。结论:远程延伸护理有助于提高透析患者治疗依从性、透析治疗充分性、患者的生活质量和护理满意度,且能减少并发症的发生。

Objective: To study the effect of nurse-led remote extended nursing model on treatment compliance,dialysis adequacy,and life quality in dialysis patients with chronic kidney disease.Methods: 148 patients diagnosed with chronic renal failure and treated with regular dialysis treatment in Baoding No.1 Hospital from January 2018 to December 2018 were selected and randomly divided into two groups according to the random number table method: routine extended nursing group and Wechat remote intervention group,with 74 cases in each group.The two groups were both treated with routine drugs and regular dialysis,the routine extended nursing group was given routine nursing,and the Wechat remote intervention group was given routine care on the basis of combined remote extended care.Cinical examination,laboratory test,and questionnaire survey were conducted on all subject,and the treatment compliance,dialysis adequacy,quality of life,nursing satisfaction,and dialysis complications were compared between the two groups.Results: After intervention,the dialysis protocol,dietary compliance,URR and KT/V,social function,physiological function,mental state,overall health score,and nursing satisfaction in the Wechat remote intervention group were significantly higher than those in the routine extended nursing group(P<0.05),and the incidence of dialysis complications was significantly lower than that in the conventional extended nursing group(P<0.05).Conclusion: Long-distance extended nursing can help to improve treatment compliance,dialysis adequacy,quality of life,and nursing satisfaction in dialysis patients,and can reduce the occurrence of complications.
