摘要
目的探讨人乳头瘤病毒(Human papilloma virus,HPV)感染对宫颈环氧合酶-2(Cyclooxgenase-2,COX-2)表达及血清前列腺素E2(Prostaglandin E2,PGE2)表达水平的影响。方法选择2017年10月-2019年5月在沈阳医学院附属中心医院就诊的宫颈癌患者200例,宫颈上皮内瘤变(Cervical intraepithelial neoplasia,CIN)患者200例,慢性宫颈炎患者200例的资料进行回顾性分析,探讨HPV感染对宫颈COX-2表达及血清PGE2表达水平的影响。结果三组患者的HPV病毒载量差异具有统计学意义[(3.56±0.59)Log值VS.(6.63±1.40)Log值VS.(10.44±2.53)Log值](P<0.001),宫颈癌组的HPV病毒载量高于CIN组,CIN组的病毒载量高于慢性宫颈炎组,三组患者的血清COX-2mRNA含量差异具有统计学意义[(4.15±0.79)Log值VS.(7.62±1.39)Log值VS.(10.45±1.05)Log值](P<0.001),宫颈癌组血清COX-2 mRNA含量高于CIN组,CIN组血清COX-2 mRNA含量高于慢性宫颈炎组。600例患者之中,HPV感染阳性435例,HPV感染阴性165例,HPV感染阳性患者的血清COX-2 mRNA含量和血清PGE2含量均高于HPV感染阴性患者(P<0.001),宫颈癌患者的血清PGE2含量和HPV病毒载量之间呈正相关(r=0.761,P<0.001),宫颈癌患者的血清PGE2含量和血清COX-2mRNA含量之间呈正相关(r=0.461,P<0.001)。结论对于HPV感染者使用HPV定量检测具有重要的意义,COX-2、PGE2和HPV感染之间具有密切的关联,对推测HPV感染的病程具有一定的指示作用,但是在实际临床工作中,还需要从患者的情况出发,进行综合考虑。
OBJECTIVE To investigate the effect of human papillomavirus(HPV) infection on the expression of cervical cyclooxygenase-2(COX-2) and serum prostaglandin E2(PGE2). METHODS 200 patients with cervical cancer, 200 patients with cervical intraepithelial neoplasia(CIN) and 200 patients with chronic cervicitis from Oct. 2017 to May. 2019 who were treated in the Affiliated Central Hospital of Shenyang Medical College were selected, and retrospectively analyzed. The effect of HPV infection on cervical COX-2 expression and serum PGE2 expression were investigated. RESULTS The difference in HPV viral load data among the three groups was significant [(3.56±0.59) Log value VS.(6.63±1.40) Log value VS.(10.44±2.53) Log value](P<0.001). The HPV viral load in the cervical cancer group was higher than that in the CIN group, and the viral load in the CIN group was higher than that in the chronic cervicitis group. The difference in serum COX-2 mRNA levels among the three groups of patients was significant [(4.15±0.79) Log value VS.(7.62±1.39) Log value VS.(10.45±1.05) Log value](P<0.001). The serum COX-2 mRNA content in the cervical cancer group was higher than that in the CIN group, and the serum COX-2 mRNA content in the CIN group was higher than that in the chronic cervicitis group. Of all 600 patients, 435 cases were positive for HPV infection and 165 cases were negative for HPV infection. The levels of serum COX-2 mRNA and serum PGE2 of HPV-positive patients were significantly higher than those of HPV-negative patients(P<0.001). There was a positive correlation between serum PGE2 content and HPV viral load in patients with cervical cancer(r=0.761, P<0.001). There was a positive correlation between serum PGE2 content and serum COX-2 mRNA content in patients with cervical cancer(r=0.461, P<0.001). CONCLUSION The HPV quantitative detection for HPV-infected patients is of great significance. There is a close correlation between COX-2, PGE2 and HPV infection, which has a certain indication for the speculative course of HPV infection. However, in actual clinical work, it is necessary to comprehensively consider the situation of the patient.
