卡前列素氨丁三醇注射液联合改良低位子宫背带式缝合术治疗前置胎盘致难治性产后出血的临床研究

Clinical study on carboprost tromethamine injection combined with modified low uterine parceled suture in the treatment of intractable postpartum hemorrhage caused by placenta previa

目的探讨卡前列素氨丁三醇注射液联合改良低位子宫背带式缝合术治疗前置胎盘致难治性产后出血的临床效果。方法选取本院2017年10月~2019年5月前置胎盘致难治性产后出血患者184例,根据治疗方案分为两组,对照组92例采用缩宫素联合“8”字间断缝合术治疗,观察组92例采用卡前列素氨丁三醇注射液联合改良低位子宫背带式缝合术治疗,比较两组手术指征、并发症、临床疗效。结果观察组术中出血量、术后24h出血量少于对照组,差异有统计学意义(P<0.05)。观察组止血时间、下床活动时间、住院时间短于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。结论卡前列素氨丁三醇注射液联合改良低位子宫背带式缝合术治疗前置胎盘致难治性产后出血的疗效显著,可减少患者出血量,降低并发症风险。

Objective To investigate the clinical study on carboprost tromethamine injection combined with modified low uterine parceled suture in the treatment of intractable postpartum hemorrhage caused by placenta previa.Methods 184 patients with intractable postpartum hemorrhage caused by placenta previa at our hospital from October 2017 to May 2019 were selected,and divided into two groups according to the therapeutic regimen.92 patients in the control group were treated with oxytocin combined with intermittent“8”word suture,while 92 patients in the observation group were treated with carboprost tromethamine injection combined with modified low uterine parceled suture.The surgical indications,complications,and clinical efficacy were compared between the two groups.Results The intraoperative bleeding volume,and bleeding volume 24h after surgery in the observation group were less than those in the control group,with statistically significant difference(P<0.05).The duration of hemostasis,out-ofbed activity and hospitalization in the observation group was shorter than that in the control group,with statistically significant difference(P<0.05).The incidence of complications in the observation group was lower than that in the control group,with statistically significant difference(P<0.05).The overall response rate in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).Conclusion Carboprost tromethamine injection combined with modified low uterine parceled suture is significantly effective in the treatment of intractable postpartum hemorrhage caused by placenta previa,which can reduce the bleeding volume of patients,and reduce the risk of complications.
