一次性根管治疗术治疗前牙慢性根尖周炎有瘘型的疗效及对患者手术前后疼痛程度的影响

Curative effects of one-off root canal therapy for treatment of chronic periapical periodontitis with fistula of anterior teethand its effect on the pain degree of patients before and after surgery

目的探讨一次性根管治疗术对前牙慢性根尖周炎有瘘型的临床疗效及对患者手术前后疼痛程度的影响。方法选取我院2016年1月~2018年7月前牙慢性根尖周炎有瘘型患者62例,随机分为对照组(30例)和观察组(32例);对照组予以传统多次性根管治疗术;观察组予以一次性根管治疗术。观察两组患者临床疗效、手术前后疼痛程度及手术相关指标情况。结果治疗后,两组患者临床疗效差异无统计学意义(P>0.05);两组患者术后咬合疼痛和自发性疼痛评分均高于治疗前,差异有统计学意义(P<0.05),但组间咬合疼痛和自发性疼痛评分差异无统计学意义(P>0.05);观察组手术费用和手术时间优于对照组,差异有统计学意义(P<0.05)。结论一次性根管治疗术治疗前牙慢性根尖周炎有瘘型的临床疗效与传统多次根管治疗术无明显差异,但前者费用低,手术时间短,值得临床推广。

Objective To explore clinical curative effects of one-off root canal therapy for treatment of chronic periapical periodontitis withfistula of anterior teeth and its effect on the pain degree of patients before and after surgery.Methods 62 patients with chronic periapical periodontitis with fistula of anterior teeth in our hospital from January 2016 to July 2018 were selected and they were randomly divided into the control group(30 cases)and the observation group(32 cases).The control group was given traditional multiple-visit root canal therapy while the observation group was givenone-off root canal therapy.The clinical curative effects,pain level before and after surgery and surgical related indexes of patients in two groups were observed.Results After treatment,the clinical curative effectof the two groups was compared,and the difference was not statistically significant(P>0.05).The postoperative occlusal pain and spontaneous pain scores were higher than those before treatment,and the difference was statistically significant(P<0.05).However,there was no statisticallysignificant difference in occlusal pain and spontaneous pain scores between the two groups(P>0.05).The operation cost and operation time of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical curative effect of one-off root canal therapy for the treatment of chronic periapical periodontitis with fistula of anterior teeth is not significantly different from that of traditional multiple root canal therapy,but the former has low cost and short operation time.It is worthy of clinical promotion.
