鼻内镜下咽鼓管球囊扩张术对慢性复发性分泌性中耳炎临床疗效及咽鼓管功能影响

Effect of eustachian tube balloon dilatation on chronic recurrent secretory otitis media and eustachian tube function under nasal endoscopy

目的探讨分析鼻内镜下咽鼓管球囊扩张术对慢性复发性分泌性中耳炎临床疗效及咽鼓管功能的影响。方法选择本院2017年1月至2017年12月收治的慢性复发性分泌性中耳炎患者94例,随机分为观察组(n=51)和对照组(n=43)。对照组采取鼓膜切开置管术治疗,观察组采取鼻内镜下咽鼓管球囊扩张术治疗,比较两组临床疗效,治疗前后咽鼓管功能障碍评分调查问卷(ETDQ-7)评分以及并发症发生情况。结果观察组患者治疗总有效率显著高于对照组(P<0.05);治疗前两组ETDQ-7评分比较差异无统计学意义,术后6个月两组ETDQ-7评分均较治疗前显著降低(P<0.05),且术后6个月观察组患者ETDQ-7评分显著低于对照组(P<0.05);两组术后并发症发生率比较差异无统计学意义。结论鼻内镜下咽鼓管球囊扩张术可有效提高慢性复发性分泌性中耳炎患者听力,有效改善患者咽鼓管功能,且安全性较高,值得临床推广。

Objective To investigate the effect of eustachian tube balloon dilatation on chronic recurrent secretory otitis media and eustachian tube function under nasal endoscopy. Methods 94 patients with chronic recurrent secretory otitis media in our hospital from January 2017 to December2017 were randomly divided into in the observation group(n=51) and the control group(n=43). The control group was treated with tympanotomy and catheterization. The observation group was treated with eustachian tube balloon dilatation under the nasal endoscope. The clinical efficacy of the two groups was compared. The questionnaire on the dysfunction of the eustachian tube was investigated.(ETDQ-7) scores and the incidence of complications. Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05). There was no significant difference in the ETDQ-7 score between the two groups before the treatment. The two groups of ETDQ-7 scores in the 6 months after the operation were significantly lower than those before the treatment(P<0.05), and the ETDQ-7 score in the observation group was significantly lower than that of the control group after the operation(P<0.05), and the two group of postoperative complications. There was no significant difference in the incidence rate. Conclusion Endoscopic balloon dilatation of the eustachian tube can effectively improve the hearing of patients with chronic recurrent secretory otitis media and effectively improve the function of eustachian tube, and it is highly safe and worthy of clinical promotion.
