股骨近端抗旋髓内钉治疗老年股骨粗隆间骨折的价值

Value of proximal femoral nail anti-rotation in the treatment of intertrochanteric fracture in the elderly

目的探讨股骨近端抗旋髓内钉(PFNa)治疗老年股骨粗隆间骨折的价值。方法选取我院2016年1月~2018年12月收治的42例老年股骨粗隆间骨折患者作为研究对象,按照随机抽样法分为观察组(n=21)和对照组(n=21)。对照组患者采用动力髋螺钉(DHS)治疗,观察组采用PFNa治疗。比较两组患者的临床治疗效果、手术相关指标及不良反应总发生率。结果观察组患者的术中出血量、术后引流量均明显少于对照组,手术时间短于对照组,差异有统计学意义(P<0.05)。观察组的治疗优良率为95.24%,明显高于对照组的66.67%,差异有统计学意义(P<0.05)。观察组的骨折愈合时间、完全负重时间以及住院时间均明显短于对照组,差异有统计学意义(P<0.05)。观察组的不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论PFNa应用于老年股骨粗隆间骨折患者的治疗中能够有效提高临床效果,降低不良反应发生风险,值得临床借鉴和应用。

Objective To investigate the value of proximal femoral nail anti-rotation(PFNa)in the treatment of intertrochanteric fracture of femur in the elderly.Methods A total of 42 elderly patients with intertrochanteric fracture of femur admitted to our hospital from January 2016 to December 2018 were selected as the study subjects.They were divided into the observation group(n=21)and the control group(n=21)according to random sampling method.The control group was treated with dynamic hip screw(DHS),while the observation group was treated with PFNa.The clinical efficacy,surgical related indicators and the total incidence of adverse reactions were compared between the two groups.Results The intraoperative blood loss,postoperative drainage volume of the patients in the observation group were significantly less than those in the control group,the operation time of the patients in the observation group was significantly shorter than that in the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of treatment in the observation group was 95.24%,which was significantly higher than that in the control group(66.67%),and the difference was statistically significant(P<0.05).The fracture healing time,total weight-bearing time and hospitalization time of the observation group were significantly shorter than those of the control group,with statistically significant differences(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion PFNa can effectively improve the clinical effect and reduce the risk of adverse reactions in the treatment of elderly patients with femoral intertrochanteric fracture,which is worthy of clinical reference and application.
